老年肌少症患者外周血白细胞及淋巴细胞亚群的分析 被引量:2

摘要 目的:测定肌少症患者外周血白细胞、淋巴细胞亚群、C-反应蛋白(CRP),并探讨其在肌少症发病机制中的作用。方法:选取2017年6月至2018年2月温州市中心医院就诊的120例老年患者,其中肌少症患者48例,非肌少症72例,分别检测外周血白细胞、淋巴细胞亚群和CRP。结果:肌少症组外周血白细胞计数为(6.7±0.9)×10~9/L,非肌少症组为(5.5±0.8)×10~9/L;肌少症组血NK细胞(CD3^-CD56^+)为(30.4±6.6)%,非肌少症组为(25.4±7.2)%;肌少症组血CRP为(6.3±1.2)mg/L,非肌少症组为(3.8±0.6)mg/L,差异均有统计学意义(P<0.05)。而2组间外周血总淋巴细胞、CD4^+T细胞、CD8^+T细胞、B细胞、NK样细胞(CD3^+CD56^+)、记忆T细胞(CD4^+CD45RO+)、初始T细胞(CD4^+CD45RA^+)、双阴T淋巴细胞(CD3^+CD4^-CD8^-)水平的差异均无统计学意义(P>0.05)。结论:肌少症患者组外周血中白细胞计数、NK细胞、CRP均高于非肌少症组,提示肌少症组患者可能存在机体微炎症和固有免疫反应过强。 Objective: To explore preliminarily the role of immune cells in pathogenesis of sarcopenia by detecting leukocyte,lymphocyte subsets and CRP of peripheral blood in patients with sarcopenia. Methods: A total of 120 old patients including 48 patients with sarcopenia and 72 patients without sarcopenia were selected in our study for the detection of leukocyte,lymphocyte subsets and CRP. Results: Leukocyte count of peripheral blood was (6.7±0.9)×10^9/L in sarcopenia group,and (5.5±0.8)×10^9/L in control group; NK cell was (30.4±6.6)% in sarcopenia group,and (25.4±7.2)% in control group; CRP was (6.3±1.2) mg/L in sarcopenia group,and (3.8±0.6) mg/L in control group. There were statistical differences between sarcopenia group and control group. However,the levels of total lymphocytes,CD4^+T cells,CD8^+T cells,B cells,NK like cells (CD^3+CD56^+),memory T cells (CD4^+CD45RO^+),naive T cells (CD4^+CD45RA^+),double negative T cells (CD^3+CD4^-CD8^-) were not significantly different between sarcopenia group and control group. Conclusion: Leukocyte count,NK cells and CRP of sarcopenia patients are higher than the controls,which indicates micro-inflammatory and hypersensitivity of innate immunity in sarcopenia patients.
