腹腔镜直肠癌根治术的路径化护理探讨

Pathway nursing for laparoscopic radical resection of rectal cancer

目的探讨路径化护理措施对腹腔镜直肠癌根治术恢复的影响。方法选取直肠癌患者150例,随机分为对照组和观察组各75例,所有患者均行腹腔镜根治手术。对照组行常规围术期护理,观察组行路径化护理措施。比较2组患者术后各恢复指标、护理满意度、生活质量评分以及并发症。结果实施路径化护理措施后,观察组通气时间、半流质进食时间、下床时间等术后各恢复指标时间均短于对照组(P <0. 05);观察组总满意率高于对照组(P <0. 05),观察组心理、角色、躯体以及社会功能等生活质量评分高于对照组(P <0. 05)。2组均出现肺部感染、肠梗阻、腹腔出血以及吻合口瘘等并发症,但观察组并发症发生率低于对照组(P <0. 05)。结论腹腔镜直肠癌根治术围术期应用路径化护理措施可有效减少术后并发症发生,改善患者生活质量,促进术后恢复,提高护理满意度。

Objective To explore the effect of pathway nursing on the recovery of rectal cancer after laparoscopic radical resection.Methods A total of 150 rectal cancer patients were randomly divided into control group and observation group,with 75 cases per group.All patients underwent radical laparoscopic surgery.The control group received routine perioperative nursing,and the observation group received pathway nursing measures.Postoperative recovery indexes,nursing satisfaction,quality of life scores and complications were compared between the two groups.Results After pathway nursing,ventilation time,semi-fluid feeding time,bed time and other postoperative recovery indicators in the observation group were shorter than that in the control group (P<0.05).The total satisfaction of the observation group was higher than that of the control group (P<0.05),and the quality of life scores in psychological,role,physical and social functions of the observation group were higher than that of the control group (P<0.05).Pulmonary infection,intestinal obstruction,abdominal bleeding,anastomotic fistula and other complications occurred in both groups,but the incidence of complications in the observation group was lower than that in the control group (P<0.05).Conclusion Pathway nursing measures in perioperative laparoscopic radical rectal cancer resection can effectively reduce postoperative complications,improve the quality of life,promote postoperative recovery,and improve nursing satisfaction.
