摘要
目的探讨心电图联合迭代重建算法CT低剂量扫描用于冠状动脉成像的临床价值。方法选取本院2017年5月至2018年8月收治的40例行冠状动脉成像患者,均行心电图联合迭代重建算法CT低剂量扫描,并以DSA检查结果为金标准计算CT低剂量扫描诊断冠状动脉粥样硬化性心脏病的敏感性、特异度、符合率及诊断狭窄程度的符合率。结果 DSA检查结果显示,40例患者中共22例(55. 00%)诊断为冠状动脉粥样硬化性心脏病; CT共诊断出23例。CT诊断冠状动脉粥样硬化性心脏病的敏感度为90. 91%(20/22),特异度为83. 33%(15/18),符合率87. 50%(35/40)。以DSA检查结果为金标准的CT低剂量扫描诊断冠状动脉狭窄程度的符合率为85. 00%(34/40)。40例患者的剂量长度乘积在82~96 m Gy·m,CT低剂量扫描的辐射剂量=0. 014×(82~96) m Gy·m,平均(1. 26±0. 61) m Gy·m。结论心电图联合迭代重建算法CT低剂量扫描用于冠状动脉成像具有较高的诊断效能,同时还可减少辐射剂量,具有较大应用前景。
Objective To explore the clinical value of electrocardiogram combined with iterative reconstruction algorithm CT low-dose scan for coronary artery imaging.Methods A total of 40 patients with coronary artery imaging admitted to our hospital from May,2017 to August 2018 were enrolled and electrocardiogram combined iterative reconstruction algorithm CT low-dose scan was performed.The sensitivity,specificity,coincidence rate of diagnosis of coronary atherosclerotic heart disease and coincidence rate of degree of stenosis were calculated by DSA examination as the gold standard.Results DSA examination showed that 22 cases(55.00%) of the 40 patients were diagnosed with coronary atherosclerotic heart disease;23 cases were diagnosed by CT.The sensitivity of CT diagnosis of coronary atherosclerotic heart disease was 90.91%(20/22),the specificity was 83.33%(15/18),and the coincidence rate was 87.50%(35/40).The coincidence rate of CT low-dose scan for the diagnosis of coronary artery stenosis with DSA results was 85.00%(34/40).The radiation dose of the CT low dose scan was(1.26±0.61mSv).Conclusion The electrocardiogram combined with iterative reconstruction algorithm CT low-dose scan for coronary angiography has high diagnostic efficiency,and can also reduce radiation dose,which has great application prospects.
