吞咽荧光透视检查对加味地黄饮子治疗卒中后吞咽障碍的疗效评价

Effect of flavored rehmannia decoction in treatment of dysphagia after stroke:Evaluated by swallowing fluoroscopy

目的观察加味地黄饮子联合吞咽训练治疗卒中后吞咽障碍的安全性和有效性。方法 2015年7月～2017年12月纳入70例卒中后吞咽障碍患者并将其随机分为对照组30例和中药组40例。对照组予吞咽训练,中药组在此基础上用加味地黄饮子,运用吞咽荧光透视检查联合吞咽功能分级量表对疗效进行评价。结果中药组临床疗效总有效率为85.0%,对照组为63.3%,中药组总有效率明显优于对照组(P<0.05)。治疗后中药组较对照组的吞咽功能分级量表评分以及VFSS评分在口腔期及总分上的差异均有统计学意义(P<0.05)。结论加味地黄饮子安全、有效,是治疗脑卒中后吞咽障碍行之有效的特色疗法;综合治疗方案更具优势,对吞咽功能有明显改善作用。

Objective To observe the safety and efficacy of flavored rehmannia decoction combined with swallowing training in the treatment of dysphagia after stroke.Methods 70 patients with dysphagia after stroke treated from July 2015 to December 2017 were selected and randomly divided into the control group(30 cases)and the TCM group(40 cases).The patients were all given swallowing training and those in the TCM group were additionally given flavored rehmannia decoction.The efficacy was evaluated by swallowing fluoroscopy and the swallowing function grading scale.Results The total effective rate of the TCM group was 85.0%,which was significantly higher than the 63.3%in the control group(P<0.05).After treatment,the scores of the swallowing function grading scale and VFSS scores in the TCM group were significantly different from those in the control group,both in the oral phase and in the total scores(P<0.05).Conclusion Flavored rehmannia decoction is safe and effective,and it is an effective treatment for dysphagia after stroke.The comprehensive treatment plan has more advantages and has obvious improvement on swallowing function.
