高依赖病房的建立

Experience in the Establishment and Management of High Dependency Unit

目的探讨开设高依赖病房(HDU)的管理体会。方法对某院2016年5月新建HDU病房5间(床位16张)至2017年5月新增HDU病房5间(床位15张)期间的护理管理经验进行总结分析。结果 HDU病房建立后共收治患者270例,床位使用率81.25%～100%,ICU床位周转率增加53.33%,患者经医治康复好转转至普通病房的达89.26%,患者及家属满意度97.13%。结论 HDU病房的建立既缓解ICU的床位紧张,便于一般危重患者(单一器官衰竭)从HDU病房安全过渡到普通病房,又便于部分病情加重的患者及时转回ICU进行抢救。其节约医院的经营成本;缩短患者在ICU的停留时间,减少并发症的发生,也能减轻患者的经济负担,保证更好的功能恢复水平;改变了护士的服务理念,提高家属的参与度与满意度,值得推广。

Objective To discuss the experience in the establishment and management of high dependency unit(HDU).Methods This paper summarized and analyzed the nursing management experience of HDU wards,with 5 units(16 beds)established in May 2016 and 5 units(15 beds)established in May 2017 in our hospital.Results A total of 270 patients were admitted to HDU since the establishment.Bed utilization rate was 81.25%~100%.ICU transferring rate increased by 53.33%.89.26%of the 270 patients were transferred to general ward after treatment and rehabilitation in HDU.Patient and family satisfaction rate reached 97.13%.Conclusion HDU relieved the pressure of bed needs of ICU and helped with patients'transferral from HDU to general ward and facilitated the transferring back to ICU of the patients with deteriorating clinical situation.As a result,management cost was decreased and ICU stay time shortened.Moreover,complication rate decreased and cost of hospitalization dropped a great deal.All in all,functional recovery improved considerably.Family members involved more in patient care and thus their satisfaction rate increased.To summarize,HDU should be promoted.
