血液透析滤过装置连续使用对透析充分性的影响

Effect of continuous use of dialysis filtration devices on dialysis sufficiency

目的探讨血液透析滤过装置使用4 h后,再次连续使用4 h对维持性血液透析(MHD)患者透析充分性的影响。方法入选2017年1月—5月我院血液净化中心透析龄≥3个月的MHD患者80例,随机分为上午组和下午组,上午组39例,下午组41例。上午组患者上机时间06:30～07:00,下午组上机时间11:30～12:00。上午组患者下机后,对血液透析滤过装置进行消毒,消毒合格后下午组患者上机,采集患者透析前、后的血标本,计算spKt/V、URR、β_2微球蛋白清除率。结果上午组spKt/V、URR、β_2微球蛋白清除率与下午组比较无统计学差异(P>0.05)。结论血液透析滤过装置使用4 h后再次连续使用4 h,不会对MHD患者透析充分性造成影响,血液透析滤过装置数量有限的医疗机构,可以采取3班或更多班次透析,以提高血液透析滤过装置使用效率,满足更多MHD患者的透析需求。

Objective To discuss the use of hemodialysis filtration device after 4 hours,effects of repeated four hours on dialysis adequacy in maintenance hemodialysis patients.Methods From January 2017 to May 2017,80 patients with maintenance hemodialysis(MHD)were randomly divided into morning group and afternoon group,39 cases in the morning group and 41 cases in afternoon group.The patients were randomly divided into the morning group and the afternoon group.The morning group of patients on the machine time 6:30 to 7:00,the afternoon group on the machine time 11:30 to 12:00,the morning after the group of patients,the hemodialysis filter device disinfection,disinfection qualified patients after the afternoon the patients were sacrificed before and after dialysis,and the spKt/V,URR andβ2 microglobulin clearance rates were calculated.Results There was no significant difference in the clearance rate of spKt/V,URR andβ2 microglobulin between the two groups(P<0.05).Conclusion Hemodialysis filtration device can be used for 4 hours after 4 hours,which will not affect the adequacy of dialysis adequacy in MHD patients.For medical institutions with limited number of hemodiafiltration devices,it can take 3 or more trips to improve the efficiency of hemodiafiltration device to meet the needs of more dialysis hemodialysis patients.
