摘要 目的研究IDH-1在胶质瘤中的表达情况及相关因素分析。方法收集郑州大学第五附属医院2015-01—2017-05手术切除经病理确诊的92例脑胶质瘤标本,按病理分级分成2组:低级别组(Ⅰ/Ⅱ) 60例,高级别组(Ⅲ/Ⅳ) 32例,其中星形胶质细胞瘤44例,少突胶质细胞瘤24例,星形-少突胶质细胞瘤12例,胶质母细胞瘤12例。男44例,女48例。伴发癫痫35例,其中低级别组28例,高级别组7例。取同期因颅脑外伤行"开颅减压术"而获取的正常脑组织20例作空白对照。结果在正常脑组织中均呈阴性表达(-),在胶质瘤组织中,高级别胶质瘤(Ⅲ/Ⅳ)中高表达(+++)为62. 5%(20/32),高于低级别胶质瘤(Ⅰ/Ⅱ)中高表达(+++)的26. 7%(16/60)(χ~2=11. 251,P=0. 000 80)。胶质瘤伴发癫痫患者中IDH1高表达(+++)为7例,不伴发癫痫中IDH1高表达为24例(χ~2=4. 147 0,P=0. 041 71)。青少年患者中高表达为50%(15/30),中年患者中高表达为23. 08%(12/52),老年患者中高表达为50%(5/10)(χ~2=7. 224,P=0. 027)。IDH1的表达程度与性别(χ~2=0. 093 0,P=0. 764 00)、病理类型(χ~2=6. 740 0,P=0. 080 66)无相关性。结论胶质瘤IDH1的表达程度与病理分级、年龄及是否伴发癫痫具有相关性。 Objective To study the expression and correlation analysis of IDH-1 in glioma.Methods A total of 92 glioma specimens diagnosed by pathology were collected from the Fifth Affiliated Hospital of Zhengzhou University from January 2015 to May 2017.They were divided into two groups according to pathological grade:60 cases of low level group(Ⅰ/Ⅱ).There were 32 patients in the high grade group(Ⅲ/Ⅳ),including 44 cases of astrocytoma,24 cases of oligodendroglioma,12 cases of astrocyte oligodendroglioma,and 12 cases of glioblastoma.There were 44 males and 48 females.There were 35 patients with epilepsy,including 28 in the low level group and 7 in the high level group.Take 20 cases of normal brain tissue obtained by"craniotomy"for craniocerebral trauma in the same period as blank control.Results Negatie expression was observed in normal brain tissue(-).In glioma tissues,high expression(+++)in high grade glioma(Ⅲ/Ⅳ)was 62.5%(20/32),higher than 26.7%(16/60)of high expression(+++)in low grade glioma(I/Ⅱ)(χ^2=11.251,P=0.000 80).In patients with glioma associated with epilepsy,high expression of IDH1(+++)was 7 cases,and high expression of IDH1 was not associated with epilepsy in 24 cases(χ^2=4.1470,P=0.041 71).High expression was 50%(15/30)in adolescents,23.08%(12/52)in middle-aged patients,and 50%(5/10)in elderly patients(χ^2=7.224,P=0.027).The degree of expression of IDH1 was not correlated with gender(χ^2=0.093 0,P=0.764 00)and pathological type(χ^2=6.740 0,P=0.080 66).Conclusion The expression of glioma IDH1 is correlated with pathological grade,age and whether it is associated with epilepsy.
