摘要
目的:检测支气管扩张合并哮喘患者中呼出气一氧化氮(FeNO)水平,并分析其与外周血EOS%、FEV_1%的相关性。方法:选取2016—2017年石河子大学第一附属医院诊断为支气管扩张合并哮喘患者37例,单纯支气管扩张患者36例,单纯哮喘患者37例,同时选取同期健康体检者25例。比较各组FeNO、外周血EOS%、使用支气管舒张剂前肺功能水平,并分析支气管扩张合并哮喘患者FeNO与外周血EOS%、FEV_1%的相关性。结果:(1)支气管扩张合并哮喘组FeNO、外周血EOS%水平显著高于单纯支气管扩张组(P <0. 01)。(2)使用支气管舒张剂前,支气管扩张合并哮喘组FEV_1、FEV_1%、FEV_1/FVC%、MMEF75/25、MMEF75/25%与单纯支气管扩张组比较差异有统计学意义(P均<0. 05),而与单纯哮喘组比较差异无统计学意义(P> 0. 05)。(3)支气管扩张合并哮喘组FeNO与外周血EOS%呈正相关(r=0. 740,P <0. 05),与FEV_1%无相关性(r=0. 191,P> 0. 05)。结论:支气管扩张合并哮喘患者FeNO、外周血EOS%水平明显偏高,使用支气管舒张剂前肺功能差。FeNO联合外周血EOS%可能对支气管扩张合并哮喘的辅助诊断有重要的参考价值。
Objective:To detect the level of fractional exhaled nitric oxide(FeNO)in patients with bronchiectasis and asthma,and to analyze its correlations with EOS%in peripheral blood and FEV1%.Methods:The clinical data of 37 patients with bronchiectasis and asthma,36 patients with bronchiectasis alone and 37 patients with asthma alone in the First Affiliated Hospital of Shihezi University was collected from 2016 to 2017.At the same time,25 cases of health examination were selected.The levels of FeNO,EOS%in peripheral blood and lung function before bronchodilator use were compared between each group,and the correlations between FeNO and EOS in peripheral blood and FEV1%were analyzed in patients with bronchiectasis and asthma.Results:(1)The levels of FeNO and EOS%in peripheral blood in bronchiectasis combined with asthma group were significantly higher than those in simple bronchiectasis group(P<0.01),however,they were not statistically different from those in simple asthma group.(2)Before using bronchodilator,there were significant differences in FEV1,FEV1%,FEV1/FVC%,MMEF75/25 and MMEF75/25%between bronchiectasis combined with asthma group and simple bronchiectasis group(all P<0.05),but there was no significant difference between bronchiectasis combined with asthma group and simple bronchiectasis group(P>0.05).(3)FeNO was positively correlated with EOS%in peripheral blood(r=0.740,P<0.05)and not correlated with FEV1%in bronchiectasis combined with asthma group(r=0.191,P>0.05).Conclusion:The levels of FeNO and EOS%in peripheral blood of patients with bronchiectasis and asthma are significantly higher,and their pulmonary functions are poor before using bronchodilator.FeNO combined with EOS%in peripheral blood may have important reference value in the auxiliary diagnosis of bronchiectasis complicated with asthma.
