米非司酮联合甲氨蝶呤治疗异位妊娠的临床效果

Clinical effect of Mifepristone combined with Methotrexate in the treatment of ectopic pregnancy

目的观察米非司酮联合甲氨蝶呤治疗异位妊娠的效果。方法选取2012年3月～2017年12月我院收治的70例异位妊娠患者作为研究对象,采用随机双盲法将其分为观察组和对照组,每组各35例。对照组患者采用米非司酮单纯治疗,观察组患者在对照组基础上联合甲氨蝶呤治疗。比较两组患者治疗前后的血人绒毛膜促性腺激素(β-HCG)值、包块大小变化,并比较两组患者的临床效果及不良反应发生情况。结果两组患者治疗前的血β-HCG值、包块比较,差异无统计学意义(P>0.05);两组患者治疗后的β-HCG值、包块均显著小于治疗前,观察组患者治疗后的β-HCG值、包块均显著小于对照组,差异有统计学意义(P<0.05)。观察组患者的治疗总有效率为94.29%,显著高于对照组的74.29%,差异有统计学意义(P<0.05)。观察组患者的不良反应总发生率为17.14%,与对照组的20.00%比较,差异无统计学意义(P>0.05)。结论采用米非司酮联合甲氨蝶呤治疗异位妊娠的效果非常显著,可有效改善患者的各种症状、体征,且不良反应少,安全性较高,值得临床推广应用。

Objective To observe the effect of Mifepristone combined with Methotrexate in the treatment of ectopic pregnancy.Methods A total of 70 patients with ectopic pregnancy admitted to our hospital from March 2012 to December 2017 were enrolled in the study.They were divided into the observation group and the control group by double-blind randomization method,with 35 cases in each group.In the control group,Mifepristone alone was used,and Methotrexate was combined in the observation group on the basis of the control group.The changes of blood human chorionic gonadotropin(β-HCG)and mass size of the two groups were compared before and after treatment,and clinical effect and adverse reactions were compared between the two groups.Results There were no significant differences in the level of blood β-HCG and mass size between the two groups before treatment(P>0.05).After therapy,the level of β-HCG and mass size of the two groups were significantly lower than those before treatment,and the level of β-HCG and mass size of the observation group were significantly lower than those of the control group,with statistically significant differences(P<0.05).The total effectiveness rate in the observation group was 94.29%,which was much higher than that in the control group accounting for 74.29%,with statistically significant difference(P<0.05).The total incidence rate of adverse reactions was 17.14% in the observation group,and compared with 20.00% in the control group,there was no statistically significant difference(P>0.05).Conclusion The effect of Mifepristone combined with Methotrexate in the treatment of ectopic pregnancy is remarkable.It can effectively improve patients′ various symptoms and signs,and has few adverse reactions with high safety.Therefore,it is worthy of clinical promotion and application.
