四妙散加味联合九味双柏膏治疗痛风性关节炎湿热痹阻证的临床效果

Clinical effect of Modified Simiao Powder combined with Jiuwei Shuangbai Ointments in the treatment of gouty arthritis identified as damp-heat obstruction pattern

目的探讨四妙散加味联合九味双柏膏治疗痛风性关节炎湿热痹阻证的临床效果。方法临床纳入2017年1月～2018年5月本院病房及门诊收治的60例患者作为研究对象,均严格按纳入标准及排除标准入组,将其随机分为研究组(30例)、对照组(30例)。对照组患者单纯采用秋水仙碱片进行治疗,研究组患者采用四妙散加味联合九味双柏膏进行治疗。观察两组患者的临床疗效及治疗前后的血尿酸、白细胞计数、红细胞沉降率等指标变化,另外,观察两组患者治疗前后的症状体征积分变化情况。结果研究组患者的临床总有效率(96.67%)显著高于对照组(83.33%),差异有统计学意义(P<0.05)。两组患者治疗后的血尿酸、白细胞计数、红细胞沉降率等指标较治疗前均明显下降,差异有统计学意义(P<0.05);治疗前,两组患者的血尿酸、白细胞计数、红细胞沉降率等指标比较,差异无统计学意义(P>0.05);治疗后,研究组患者的血尿酸、白细胞计数、红细胞沉降率等指标均显著低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后的关节疼痛、肿胀、压痛等症状体征积分较治疗前均明显降低,差异有统计学意义(P<0.05);治疗前,两组患者的关节疼痛、肿胀、压痛等症状体征积分比较,差异无统计学意义(P>0.05);治疗后,研究组患者的上述症状积分均显著低于对照组,差异有统计学意义(P<0.05)。结论采用四妙散加味联合九味双柏膏对痛风性关节炎湿热痹阻证患者进行治疗,效果理想,可进一步提高临床疗效,降低血尿酸水平,改善患者临床症状及体征,具备临床实施意义与价值。

Objective To investigate the clinical effect of Modified Simiao Powder combined with Jiuwei Shuangbai Ointments in the treatment of gouty arthritis identified as damp-heat obstruction pattern.Methods From January 2017 to May 2018,60 patients in the inpatient wards and outpatient clinics in our hospital were enrolled in the study.Following the strict inclusion and exclusion criteria,all patients were divided into study group(30 cases)and control group(30 cases)in random.In the control group,Colchicine Tablets alone was used,while in the study group,Modified Simiao Powder combined with Jiuwei Shuangbai Ointments was adopted.The clinical efficacy,and the changes of blood uric acid,white blood cell count and erythrocyte sedimentation rate before and after treatment were observed.Inaddition,the changes of the scores of symptoms and signs were observed before and after treatment.Results The total effectiveness rate of the study group was 96.67%,significantly higher than that of the control group accounting for 83.33% with a statistical difference(P<0.05).The blood uric acid,white blood cell count and erythrocyte sedimentation rate after treatment decreased greatly(P<0.05),while before treatment,the indexes of blood uric acid,white blood cell count and erythrocyte sedimentation rate between the two groups were not displayed statistical significance(P>0.05).After treatment,the blood uric acid,white blood cell count and erythrocyte sedimentation rate in the study group were significantly lower than those in the control group with statistical significance(P<0.05).The scores of symptoms and signs such as joint pain,swelling and pressing pain significantly reduced compared with those before treatment(P<0.05),while there was no significant difference in the scores of symptoms and signs including joint pain,swelling,and pressing pain(P>0.05).After treatment,the above symptom scores of the study group were significantly lower than those of the control group with statistical significance(P<0.05).Conclusion Application of Modified Simiao Powder combined with JiuweiShuangbai Ointments in the treatment of patients with gouty arthritis identified as damp-heat obstruction pattern obtains an optimal effect,which can further enhance clinical efficacy,reduce the blood uric acid level,and improve the clinical symptoms and signs.It has great significance and value on implementation in clinical practice.
