摘要
目的探讨住院医师规范化培训中的继续教育课程设置对住院医师综合能力的积极作用。方法根据岗位胜任力需求与培训细则的要求,探讨在规范化培训中住院医师应具备的知识及技能。2015年8月~2017年8月,进行为期2年的相应课程设置,通过问卷调查了解住院医师对培训课程的满意度,以及课程前后对住院医师进行难度相当的测验,了解成绩提升情况。结果培训课结束后的当堂测试成绩均较培训前有所提升。合格率(≥60分)提高前三位的依次是影像学(28%)、心电图(26%)、超声检查(26%)。优秀率(>80分)提高前三位的依次是沟通技巧(35%)、心肺复苏(32%)、外科技能(28%)。住院医师对课程设置总体满意度:非常满意为85%,比较满意为15%,不满意为0%。在对课程有新建议方面,63%的住院医师对现有课程表示无新建议,22%的住院医师建议提供关于论文撰写、科研设计方面的课程。15%的住院医师建议培训时内外科住院医师的内容有所区分,适当偏重于内科或外科系统。结论住院医师在规范化培训中的继续教育课程设置使住院医师在临床技能及人文素养方面的考核成绩均有所提升。课程设置可进行进一步优化,完善分级设置,以高质量地完成住院医师的培训,使其临床技能得到进一步提升。
Objective To explore the positive effect of continuing education curriculum setting on the comprehensive ability in standardized training of residents.Methods According to the requirements of job competency and training rules,the knowledge and skills that residents should possess in standardized training were discussed.A two-year course from August 2015 to August 2017 was offered.The residents′ satisfaction degree with the training courses was investigated by questionnaires,and the residents were tested before and after the course so as to know the improvement of the results.Results At the end of the training course,the test scores were improved compared with those before the training.The qualified rate(≥60 points)was increased by 28% in imaging,26% in electrocardiogram and 26% in ultrasonography.The best rate(>80 points)was 35% in communication skills,32% in cardiopulmonary resuscitation and 28% in surgical skills.The overall satisfaction degree of the residents in the curriculum setting was 85%(very satisfied),15%(satisfied)and 0%(dissatisfied)respectively.In terms of new proposals for the curriculum,63% of residents said they had no new proposals for the existing curriculum,and 22% suggested offering courses on paper writing and research design.15% of the residents suggested that the contents of the training should be differentiated between the physicians and the surgeons,with an appropriate emphasis on the internal medicine or surgical system.Conclusion The curriculum setting of continuing education for in standardized training of residents can improve the assessment results of residents in clinical skills and humanistic literacy.The curriculum can be further optimized and improve the grading settings to complete the residents′ training of high quality,so as to further improve their clinical skills.
