内镜和CT评估病变程度在分析胃癌患者疗效中的价值

Value of endoscopy and CT for assessment of the efficacy of gastric cancer patients

目的研究内镜和CT评估病变程度在分析胃癌患者疗效中的价值。方法纳入2006年5月至2014年5月在延安大学附属医院接受化疗的胃癌患者189例,开展内镜和CT疗效评估,统计分析胃癌患者总生存期(OS)和无进展生存期(PFS)。结果 CT和内镜疗效评估有效者分别为54例(28. 6%)和93例(49. 2%)。内镜有效者OS和PFS显著高于CT有效者(P <0. 0001 vs P=0. 01; P <0. 0001 vs P=0. 008)。在接受新辅助化疗的胃癌患者中上述结果更加显著(P <0. 0001 vs P> 0. 05; P <0. 0001 vs P>0. 05)。多变量生存分析显示,与CT无效者相比,内镜无效者为独立的预测因素,内镜无效者OS更差。结论内镜评估可有效预测胃癌化疗患者预后,有效性高于CT。

Objective To investigate the value of endoscopy and CT for assessment of the efficacy of gastric cancer.Methods A total of 189 patients with gastric cancer undergoing chemotherapy in the Affiliated Hospital of Yan’an University from May 2006 to May 2014 were enrolled in this study,and the chemotherapy efficacy was assessed by endoscopy and CT.The overall survival(OS)and progression-free survival(PFS)of the patients were statistically analyzed.Results The responders assessed by endoscopy and CT were 54 cases(28.6%)and 93 cases(49.2%),respectively.The OS and PFS of responders assessed by endoscopy were significantly higher than those of CT responders(P<0.0001 vs P=0.01;P<0.0001 vs P=0.008).The above results were more pronounced in gastric cancer patients receiving neoadjuvant chemotherapy(P<0.0001 vs P>0.05;P<0.0001 vs P>0.05).Multivariate survival analysis showed that compared with non-responder assessed by CT,the non-responder assessed by endoscopy was an independent predictor and the non-responder assessed by endoscopy had worse OS.Conclusion Endoscopic assessment can effectively predict the prognosis of patients with gastric cancer chemotherapy,the effectiveness is higher than CT.
