从卵子形态及胚胎质量选择移植胚胎的研究进展

摘要 体外受精-胚胎移植技术(ART)的成功取决于该过程中每个独立步骤发挥其最大效能。控制性超排卵、取卵技术及胚胎培养条件的改善确保了较高的受精率和可移植胚胎的数目,然而胚胎质量始终是影响妊娠结局的主要因素之一。体外受精(IVF)和卵胞浆内单精子显微注射(ICSI)周期选择2~3枚胚胎移植可提高妊娠率,但多胎率明显增加,有报道多胎率高达50%[1],而且母亲及胎儿妊娠期风险增加。因此近年来许多中心开展单胚胎移植技术,希望通过减少胚胎移植数目,降低多胎率。但若不能预测单个胚胎的发育潜能,盲目减少移植胚胎数目,妊娠率必然降低。因此,对胚胎发育潜能的早期、准确预测,选择高发育潜能的胚胎移植可提高妊娠率、种植率,降低多胎率。本文从IVF/ICSI过程中卵母细胞形态(D0)、原核期胚胎(D1)、卵裂期胚胎(D2、D3)和囊胚期胚胎(D5)形态学评分进行综述。
