不同麻醉方式在分娩镇痛中的应用效果

Clinical effect of different anesthesia methods in labor analgesia

目的探索不同麻醉方式在分娩镇痛中的应用效果。方法将医院产科收治的324例产妇按不同麻醉方式分为静脉麻醉组(142例,静脉麻醉)与椎管内麻醉组(182例,椎管内麻醉)。比较两组产妇的麻醉效果。结果静脉麻醉组产妇的NRS评分、镇痛起效时间、维持时间、泌乳素水平、产后出血、腰背痛发生率及Bromage评分均优于椎管内麻醉组(P<0.05)。静脉麻醉组新生儿心动过缓发生率均低于椎管内麻醉组,NACS评分高于椎管内麻醉组(P<0.05)。结论静脉麻醉在产妇分娩镇痛中的应用效果优于椎管内麻醉,能缓解疼痛,改善母婴结局,值得推广。

Objective To explore the application effect of different anesthesia methods in labor analgesia.Methods A total of 324 parturients admitted to obstetrics department in our hospital were divided into intravenous anesthesia group (142 cases,intravenous anesthesia) and intraspinal anesthesia group (182 cases,intraspinal anesthesia) according to the different anesthesia methods.The anesthesia effects of the two groups were compared.Results The NRS score of parturients,the onset time and maintenance time of analgesia,level of prolactin,the incidences of postpartum hemorrhage and low back pain and Bromage score in the intravenous anesthesia group were better than those in the intraspinal anesthesia group (P<0.05).The incidence of neonatal bradycardia in the intravenous anesthesia group was lower than that in the intraspinal anesthesia group,and the NACS score was higher than that in the intraspinal anesthesia group (P<0.05).Conclusion Intravenous anesthesia is superior to intraspinal anesthesia in the analgesia of parturient labor.It can relieve pain and improve maternal and infant outcomes,which is worth popularizing.
