腹腔镜手术与开腹手术对宫外孕术后受孕率的影响

Effect of laparoscopic surgery and laparotomy for the pregnancy rate after surgery in ectopic pregnancy

目的探讨腹腔镜手术与开腹手术对宫外孕术后受孕率的影响。方法将医院接诊的宫外孕手术患者40例按照手术方式的不同分为腹腔镜组和开腹手术组,各20例。比较两组患者的手术时间、术中出血量、下床活动时间以及妊娠情况。结果腹腔镜组手术时间和下床活动时间均短于开腹手术组,且腹腔镜组术中出血量少于开腹手术组(P<0.05);腹腔镜组宫内受孕率高于开腹手术组,且宫外受孕率低于开腹手术组(P<0.05);两组患者未受孕率比较,差异不显著(P>0.05)。结论与传统开腹手术相比,腹腔镜手术治疗宫外孕具有微创、安全的特点,而且手术时间短,疼痛轻,恢复快,患者再次妊娠时出现宫外孕的概率小。因此腹腔镜手术在宫外孕且有生育要求的患者中有重要作用,可以广泛应用于宫外孕的临床治疗,提高患者的生育率。

Objective To explore the effect of laparoscopic surgery and laparotomy for the pregnancy rate after surgery in ectopic pregnancy.Methods Forty patients receiving ectopic pregnancy surgery in the hospital were divided into laparoscopic group and laparotomy group according to different surgery methods,with 20 cases in each group.The operation time,intraoperative bleeding volume,ambulation time and intrauterine pregnancy were compared between the two groups.Results The operation time and ambulation time in the laparoscopic group were shorter than those in the laparotomy group,and the amount of intraoperative bleeding in the laparoscopic group was less than that in the laparotomy group(P<0.05).The intrauterine pregnancy rate of the laparoscopic group was higher than that of the laparotomy group,and the extrauterine pregnancy rate of the laparoscopic group was lower than that of the laparotomy group (P<0.05).There was no significant difference in the rates of non-conception between the two groups (P>0.05).Conclusion Compared with traditional laparotomy,laparoscopic surgery for ectopic pregnancy has the characteristics of minimally invasive,safe,with shorter operation time,less pain,faster recovery,and less probability of ectopic pregnancy during pregnancy.Therefore,laparoscopic surgery plays an important role in patients with ectopic pregnancy and fertility requirements.It can be widely used in clinical treatment of ectopic pregnancy and improve the fertility rate of patients.
