HAART治疗后AIDS患者的中医体质类型、舌面脉特点及体质类型和舌面脉对治疗影响的分析

TCM Constitution and Characteristics of Tongue Pulse and Their Effects on the Therapies in AIDS Patients after HAART

目的分析探讨接受高效反转录病毒治疗(HAART)治疗的获得性免疫缺乏综合征(AIDS)患者中医体质类型和舌脉特点及其对HAART后免疫应答的影响,为开展AIDS中医药治疗提供参考。方法对2015年1月1日至2017年12月31日在广州市第八人民医院门诊接受免费HAART治疗1年的AIDS患者,采用舌面脉中医体质辨识系统中医体质辨识系统DS01-A型舌面脉信息采集系统采集舌脉数据,应用频数分析、Logistic分析等统计学方法分析不同免疫应答效果患者的中医体质及舌面脉特点。结果共有139例患者纳入研究,根据其免疫应答反应分为免疫无应答组和免疫重建组,其中免疫无应答组83例,前3位中医体质为湿热质31例,占37.35%,气虚质和痰湿质各13例,占15.66%;免疫重建组56例,前3位中医体质类型为湿热质20例,占35.71%,气虚质19例,占33.93%,平和质6例,占10.71%。两组患者的中医体质差异无统计学意义(P> 0.05),而两组的年龄差异有统计学意义(P <0.05)。研究发现面色红、舌体瘦、脉弦、痰湿质患者更常见于免疫无应答患者(P <0.05)。结论应重视及早开始HAART,可根据患者的中医体质针对性地开展体质调养,对免疫无应答患者可运用健脾疏肝、祛湿化痰方法进行调养。

Objective:To analyze TCM constitution and characteristics of tongue pulse in AIDS patients after HAART,and the influence on the immune response after HAART,providing reference for the development of traditional Chinese medicine treatment of AIDS.Methods:After one-year HAART,DS01-A was used to collect data of tongue pulse in AIDS patients and frequency analysis,logistic analysis and other statistical methods were used to analyze TCM constitution and characteristics of tongue pulse in patients with different immune response effects.Results:139 patients were enrolled in the study.According immune response,they were divided into immune non-response(INR)group and immune response group.In INR group(83 cases),31 cases(37.35%)had damp-heat constitution,13 cases(15.66%)Qi deficiency constitution and 13 cases(15.66%)phlegm-dampness constitution.In the immune response group,there were 56 cases,20 case(35.71%)of damp-heat constitution,19 cases(33.93%)of Qi deficiency constitution,6 cases(10.71%)of calm constitution.There was no significant difference in TCM constitution between two groups(P >0.05),while the difference in age was statistically significant(P<0.05).Red face,thin tongue and intense pulse were more common in INR group(P<0.05).Conclusion:It is important to start HAART as early as possible,and the physical conditioning can focus on the treatment of clearing heat,regulating liver,invigorating-spleen and despeling phlum.
