老年多器官功能不全综合征急性肾损伤的中医辨治理论探究

Exploration of TCM Differentiation and Treatment of Acute Kidney Injury in Senile Multiple Organ Insufficiency Syndrome

老年多器官功能不全综合征(MODSE)患者急性肾损伤是临床较为常见的急危重症,由发病人群体质的特殊性决定了其病因病机的独特性。本文基于"云南陆氏气阴学术流派"的经验传承,认为以气阴亏虚为疾病发生的起点,以浊毒内停、肾络瘀阻为疾病发展的核心,并由此提出以解毒化浊、益气养阴、活血祛瘀为治疗老年多器官功能不全综合征急性肾损伤的方法,以期能达到改善患者肾功能,遏制"诸脏衰"的病势。并为中西医结合治疗MODSE提供思路及方法。

Acute kidney injury(AKI)in elderly patients with multiple organ dysfunction syndrome(MODSE)is a common acute and severe disease in clinic.The unique etiology and pathogenesis of AKI is determined by the physical characteristics of the patients.Based on the experience of“Lushi Qiyin Academic School in Yunnan”,this paper holds that deficiency of Qi and Yin is the start of disease occurrence;turbidity and toxicity and stagnation of kidney collaterals are the core of disease development.Thereby,the methods of detoxification and turbidity elimination,Qi-enriching and Yin-nourishing,blood-activating and blood-stasis-removing were put forward to treat AKI in elderly patients with MODSE,aimed at improving the renal function of patients and curb the condition of“various organs failure”.It also provides ideas and methods for the treatment of MODSE with integrated traditional Chinese and Western medicine.
