胃癌患者应用消化道重建手术治疗的临床效果及对患者胃功能的影响

Clinical Effect of Gastrointestinal Reconstruction Surgery on Gastric Cancer Patients and Its Effect on Gastric Function

目的探究消化道重建手术应用于胃癌患者治疗的临床效果及对患者胃功能的影响。方法该次80例实验对象方便选自2015年10月—2018年2月期间该院接收的胃癌患者,按照随机表发均分成实验组(n=40)和对照组(n=40),实验组应用功能性间置空肠代胃术进行治疗,对照组则应用Roux-en-Y术进行治疗,将两种治疗效果对比分析。结果实验组的体重增加量(3.01±0.31)kg、每日进食量(341.21±45.78)g、每日进食频率(3.11±0.74)次,均优于对照组,组间差异有统计学意义(t=15.133、8.404、7.844;P=0.000);实验组的排便时间(37.51±6.74)h、排气时间(30.14±4.56)h、肠鸣音恢复时间(29.78±9.74)h、住院时间(8.78±1.74)h、手术用时(2.67±0.44)h,均低于对照组,组间差异存有统计学意义(t=11.823、10.459、3.454、3.895、9.210,P=0.000);实验组在术后生活质量评分中的生活感受(1.55±0.41)分、家庭支持(1.67±0.39)分、健康感受(1.24±0.47)分、活动能力(1.47±0.31)分、日常生活(1.57±0.38)分,均高于对照组,组间差异有统计学意义(t=5.412、2.310、5.079、3.801、P=0.000、0.024、0.000, 0.000);在并发症发生率上,实验组低于对照组(7.50%vs 25.00%),组间差异有统计学意义(χ2=5.165,P=0.023)。结论胃癌患者应用功能性间置空肠代胃术效果确切,能够显著提高患者胃功能和生活质量,且并发症较少,具有一定安全性。

Objective To investigate the clinical effects of digestive tract reconstruction surgery on gastric cancer patients and its effect on gastric function.Methods All the 80 subjects were convenient selected from patients with gastric cancer who were admitted to the hospital from October 2015 to February 2018.They were divided into experimental group (n=40) and control group (n=40) according to randomized samples.The experimental group was treated with functional interstitial jejunal surgery,and the control group was treated with Roux-en-Y.The two treatment effects were compared and analyzed.Results The weight gain of the experimental group (3.01±0.31)kg,the daily intake (341.21±45.78)g,and the daily feeding frequency (3.11±0.74)times were better than the control group.There was a statistically significant difference between the two groups,(t=15.133,8.404,7.844,P=0.000); defecation time of the experimental group (37.51±6.74)h,exhaust time (30.14±4.56)h,recovery time of bowel sound (29.78±9.74)h,hospitalization time (8.78±1.74)d,and operation time(2.67±0.44) h,all were lower than the control group,the difference between the groups was statistically significant(t=11.823,10.459,3.454,3.895,9.210,P=0.000); the life experience of the experimental group in the postoperative quality of life score (1.55±0.41)points,support(1.67±0.39) healthy feeling (1.24±0.47)points,activity ability (1.47±0.31)points,daily life (1.57±0.38)points,were higher than the control group,and the difference between the groups was statistically significant (t=5.412,2.310,5.079,3.801,P=0.000,0.024,0.000,0.000); in the incidence of complications,the experimental group was lower than the control group (7.50% vs 25.00%),the difference between the groups was significant different (χ^2=5.165,P=0.023).Conclusion The application of functional interstitial jejunal gastrostomy in patients with gastric cancer is effective,which can significantly improve the gastric function and quality of life of patients with less complications and safety.
