白内障现代囊外摘除人工晶体植入术的疗效分析

Clinical Analysis of Modern Extracapsular Cataract Extraction and Intraocular Lens Implantation

目的对比分析小切口现代囊外摘除人工晶体植入术与传统现代囊外摘除人工晶体植入术治疗白内障的临床效果。方法选取2012年6月-2013年12月在我院进行手术治疗的白内障患者109例作为研究对象,按照治疗方法不同分为观察组和对照组,其中观察组55例,采用小切口现代囊外摘除人工晶体植入术;对照组54例,采用传统的现代囊外摘除人工晶体植入术。观察并比较治疗后两组的视力水平、眼压和患者满意度。结果观察组患者术后1个月视力矫正情况与对照组相当,秩和检验显示,两组间的差异无统计学意义(Z=-0.383,P=0.702)。观察组出现眼压增高、角膜水肿、后囊破裂和悬韧带离断等并发症的发生率均低于对照组,差异具有统计学意义(χ~2=4.127,P=0.042)。两组患者术前眼压对比无差异(P>0.05),而术后对比眼压明显有升高的趋势,与术前对比存在明显差异,两组数据对比具有统计学意义(P<0.05);观察组患者术后2～24 h,眼压较对照组明显升高,差异具有统计学意义(P<0.05);但是术后48 h两组患者均与术前相当,差异无统计学意义(P>0.05);两组患者在术前与术后散光情况的对比中,观察组与对照组对比差异无统计学意义(P>0.05)。结论小切口现代囊外摘除人工晶体植入术治疗白内障患者,创伤小,术后并发症发生率低,术后能够明显改善患者的视力,具有很好的临床疗效。

Objective To compare and analyze the clinical effect of modern small-incision extracapsular cataract extraction with intraocular lens implantation and that of traditional modern extracapsular intraocular lens implantation in the treatment of cataracts.Methods One hundred and nine cataract patients who underwent surgical treatment in our hospital between June 2012 and December 2013 were divided into the observation group(n=55)and control group(n=55).The observation group was treatd with modern small-incision extracapsular cataract extraction with intraocular lens implantation while the control group received traditional modern extracapsular cataract extraction with intraocular lens implantation.Visual acuity,IOP and patient satisfaction were observed and compared after treatment.Results The corrected visual acuity in the observation group was similar to that in the control group one month after operation.The rank sum test showed that there was no significant difference between the two groups.The incidence of intraocular pressure,corneal edema,posterior capsular rupture and rupture of the suspended ligament in the observation group was lower than that in the control group,and the incidence of complications in the observation group was significantly lower than that in the control group(χ^2=4.127,P=0.042).There was no significant difference in intraocular pressure between the two groups(P>0.05),but there was a significant increase in intraocular pressure in both groups after operation(P>0.05).The intraocular pressure in the observation group was significantly higher than that in the control group(P<0.05).However,48 hours after operation,there was no significant difference between the two groups(P>0.05),and the case was the same with preoperative and postoperative astigmatism(P>0.05).Conclusion Small incision modern extracapsular cataract extraction with intraocular lens implantation in the treatment of cataracts is less traumatic and complicated after operation.The incidence of complications is low,the postoperative visual acuity can be significantly improved,and theclinical effect is good.
