单纯冠心病合并高尿酸血症患者的临床特征及冠状动脉病变特点分析

Clinical and coronary arterial characteristics of simple coronary heart disease complicating hyperuricemia

目的探讨单纯冠心病合并高尿酸血症患者的临床特征及冠状动脉病变特点。方法将72例冠心病患者根据是否合并高尿酸血症分为研究组(合并高尿酸血症)和对照组(无高尿酸血症),各36例。收集患者入院24 h内的实验室检查结果、冠脉造影检查结果及不良事件发生情况。结果研究组患者冠状动脉单支病变发生率为19.44%,低于对照组的38.89%(P<0.05);三支病变发生率为36.11%,高于对照组的13.89%(P<0.05)。研究组Gensini评分、总胆固醇和血尿酸水平及不良事件发生率均高于对照组(P<0.05或P<0.01)。结论与对照组相比,研究组患者总胆固醇和血尿酸水平较高,冠状动脉三支病变的发生率高,面临的风险更大。提示,高尿酸血症可作为预测冠心病病变程度和发生主要心血管事件的因素。

Objective To discover the clinical features of coronary artery disease(CHD)concurrent with hyperuricemia(HUA).Methods Seventy-two cases of coronary heart disease were divided into experimental group(with HUA)and control group(non-HUA)according to whether the patients were complicated with HUA or not.Collected the datum of laboratory determination and coronary angiography within 24 hours of admission.Results The experimental group(19.44%)had lower incidence of single pathological coronary artery compared with the control group(38.89%).The incidence rate of three-vessel lesion(36.11%)in the experimental group existed higher percentage than that of the control one(13.89%,P<0.05).The experimental group achieved higher Gensini score compared with the control group(P<0.01).The serum total cholesterol,uric acid concentrations of the experimental group were statistically higher than those of the control one(P<0.01).The incidence of adverse event occurred significantly more often in the experimental group than in the control group(P<0.05).Conclusion Compared with non-high uric acid syndrome patients,CHD with HUA had higher levels of serum total cholesterol,higher incidence of three-vessel coronary lesions,and a greater risk for major adverse cardiovascular event.The current study suggests that the SUA levels may be a predictor for incidence of major cardiovascular events or severity of coronary lesion in coronary artery disease with HUA.
