双固定锚钉系统微创治疗成人胫骨结节撕脱骨折9例疗效分析

Minimally invasive surgical treatment with Twinfix Ti for avulsion fracture of tibial tubercle in 9 adults

目的探讨9例成人胫骨结节撕脱骨折的微创切开复位双固定锚钉系统固定的治疗效果。方法回顾性的分析本科室从2011年9月至2017年2月中的9例成人胫骨结节撕脱骨折的临床特点,使用微创切开复位骨折,用双固定锚钉系统固定治疗。9例患者依据Ogden分型,ⅠB型3例,ⅡA型5例、ⅢA型1例;其中男8例,女1例,左侧7例,右侧2例,年龄23～45岁,平均年龄33.6岁;随访时间为9～18个月,平均13.7个月。评价患者的伤口愈合情况、骨折愈合情况、随访X线表现、术后膝关节HSS(hospital forspecial surgery knee score)评分。结果全部手术切口均甲级愈合,骨折均获得一期愈合,末次随访膝关节屈曲度平均125°,HSS评分都为优,平均(95.0±0.1)分。结论微创切开复位并用双固定锚钉系统固定治疗成人胫骨结节撕脱骨折可获得良好的治疗效果。

Objective To investigate the therapeutic effect of fracture reduction by minimally invasive open incision with Twinfix Ti in 9 cases of avulsion fracture of tibia tubercle.Methods Nine cases of adult tibial tubercle avulsion fracture from September 2011 to February 2017 were enrolled.The clinical characteristics and treatment effect of fracture reduction by minimally invasive incision with Twinfix Ti fixation were analyzed.According to the Ogden classification,there were 3 cases of Ib,5 cases of IIa and 1 case of IIIa.Eight were male and one was female,while 7 were on the left and 2 were on the right.The age ranged from 23 to 45,with an average number of 33.6 years.The follow-up time was from 9 months to 18 months,with an average time of 13.7 months.Results All of the fractures were healed in the first stage,and the mean range of motion of knee joint was 125 degrees at the last follow-up,while the HSS scores were all excellent.Conclusion The treatment of avulsion fracture of tibial tubercle in adults with reduction by minimally invasive incision and Twinfix Ti could achieve good therapeutic effect.
