双髓内钉技术治疗胫腓骨干双骨折的临床分析

Clinical analysis of double intramedullary nail in treatment of tibiofibular shaft fractures

目的本研究对胫腓骨干双骨折运用双髓内钉闭合复位固定技术的临床效果进行评价。方法选取2015年5月至2017年5月于本院就诊的27例胫腓骨干双骨折患者行闭合复位双髓内钉固定术,评价术后骨折愈合情况。结果所有患者均有效随访,均在术后4～6个月内达到临床愈合,术后12～18个月内达到骨性愈合,其中22例患者术后愈合评价优,5例患者评价良,无患者出现预后评价差。结论双髓内钉技术治疗胫腓骨干双骨折属于微创手术,对骨折局部的软组织和血供保护具有优势,骨折内固定坚强,创造稳定的愈合环境,且较少出现局部并发症。可以避免骨折愈合延迟或骨不连,增高了愈合率、缩短愈合时间,是一种安全、有效的治疗方法。

Objective To evaluate the clinical effect of closed reduction and fixation with double intramedullary nails for tibiofibular shaft fractures.Methods From May 2015 to May 2017,27 patients with tibiofibular shaft fractures were treated with closed reduction and double intramedullary nailing.Results All the cases were followed up effectively.Clinical healing was achieved in all the cases within 4-6 months and bone healing within 12-18 months after operation.Among them,22 patients had excellent healing evaluation,5 patients had good healing evaluation and no patients had poor prognosis.Conclusion Double intramedullary nailing is a minimally invasive technique for the treatment of tibiofibular shaft fractures.It has advantages in protecting the soft tissue and ensuring blood supply of the fracture area with rigid internal fixation,a stable healing environment,and few local complications.It is a safe and effective treatment as it can avoid delayed fracture healing or nonunion,increase the healing rate and shorten the healing time.
