双侧上颌窦底提升种植修复10年后病例回顾

摘要 目的通过对采用单一骨粉行双侧上颌窦底提升植骨种植修复10年后进行回顾研究,评估其远期临床效果。方法选取大连市口腔医院种植科2007年5月~2008年12月进行的单一骨粉双侧上颌窦底提升植骨手术患者6例共21枚种植体的临床资料回顾分析,选取种植术后及10年后影像学检查、口内检查及ISQ检查。测量种植体边缘骨水平(CBL)、边缘骨吸收(MBL)、根尖骨高度(ABH)及根尖骨吸收(ABL);改良菌斑指数(mPLI)与改良出血指数(mSBI);计算种植体存留率,统计数据并进行描述性分析。结果 6例患者共21枚种植体10年后复查,存留率为100%,总体ISQ值在70~82之间;MBL为(2.76±0.43)mm,ABL为(0.53±0.24)mm。21枚种植体软组织指数mPLI为1.51±0.61,mSBI为1.31±0.74,其中有4枚种植体出现种植体周围黏膜炎,其余患者口腔卫生较好。结论双侧上颌窦底提升植骨种植修复10年后临床复查,种植体均能正常行使咀嚼功能,10年存留率100%。使用单一骨粉行上颌窦底提升植骨术10年远期临床效果较佳,成骨效果满意。 Objective The long-term clinical effect was evaluated by retrospective study of double-sided maxillary sinus floor elevation with a single bone powder and implant repair .Methods Collect clinical data of 6 patients with a total of 21 implants in a single bone powder bilateral maxillary sinus floor bone graft surgery from May 2007 to December 2008 in Dalian Department of Stomatology were retrospectively analyzed.By digital panoramography after implantation,imaging 、 intraoral and ISQ examination 10 years later,measurement of crest bone level (CBL);marginal bone loss (MBL);apical bone height (ABH);apical bone loss (ABL);modified plaque index (mPLI)) and modified bleeding index (mSBI);implant retention rate .Statistical data and descriptive analysis. Results A total of 21 implants in 6 patients were reviewed after 10 years,and the retention rate was 100%.The ISQ count is between 70 and 82.MBL is (2.76 ± 0.43) mm;ABL is (0.53 ± 0.24) mm.The soft tissue index includes mPLI is 1.51 ± 0.61 and the mSBI is 1.31 ± 0.74.There are 4 implants with peri-implant mucositis.Others oral hygiene of the patients is well.Conclusion After 10 years of bilateral maxillary sinus floor elevation and bone grafting,all implants can perform masticatory function normally,and the 10-year retention rate is 100%.The use of a single domestic bone powder for maxillary sinus floor elevation has a good long-term clinical effect and satisfactory osteogenesis.
