类风湿关节炎的中医药研究进展

Research progress of traditional Chinese medicine on rheumatoid arthritis

类风湿性关节炎(rheumatoid arthritis,RA)是临床常见自身免疫疾病,可导致患者关节畸形,功能丧失,严重影响患者生活质量。传统中医对本病认识历史悠久,近现代医家认为,RA病机特点为本虚标实,肝肾亏虚、脾胃气血亏虚、阳气亏虚、营卫不和等正气不足为RA发病之本,风寒湿热、痰浊、瘀血痹阻等内外之邪阻滞经络为RA发病之标,治疗方面提出了"从五脏论治""从毒邪论治""从络论治"以及以现代中药药理为用药指导的论治思想。实验研究证实了中药复方治疗RA的有效性,并从缓解RA滑膜炎症,抑制滑膜增生和骨破坏等现代医学理论方面揭示了中药复方的作用机制。

Rheumatoid arthritis(RA)is a common clinical autoimmune disease,which can lead to joint deformity,loss of function,and seriously affects the quality of life of the patients.Traditional Chinese medicine has a long history of understanding this disease.The pathogenesis of RA is characterized by deficiency in origin and excess in superficiality.Liver and kidney deficiency,spleen,stomach,qi and blood deficiency,yang qi deficiency,and disharmony between ying and wei are the root causes of RA pathogenesis.The blocking of meridians by internal and external evils such as wind-cold-damp-heat,phlegm turbidity,and blood stasis obstruction is manifestation cause of RA pathogenesis.In the aspect of treatment,the thought of“treating based on five zang-organs”,“treating based on toxin evil”,“treating based on collaterals”and“treating with modern Chinese medicine pharmacology as the guidance of medication”is put forward.Experimental studies have confirmed the effectiveness of Chinese herbal compound in treating RA and revealed the mechanism from the modern medical theories such as alleviating RA synovial inflammation,suppressing synovial hyperplasia and bone destruction.
