应用成本性态分析提高医疗检查效率:以消化内镜为例

Application of Cost Behavior Analysis to Improve the Efficiency of Medical Examination:Taking Digestive Endoscopy for Example

目的在有限资源条件下促进工作量增长,降低单位成本,提高医院经济效益。方法以消化内镜检查为例,应用成本性态分析,通过区分固定成本与变动成本,对场地、设备、人力等各项成本进行分类归集,并在充分分析使用情况的基础上,制定和实施资源调配、绩效管理、信息化建设等管理措施。结果工作量大幅上升,2017年上半年同比增幅达31.56%;预约时间明显缩短,普通检查为1天～2天,麻醉检查为2天～5天;术前平均住院日由原来的5.64天下降至3.9天。结论成本性态分析具有良好适用性,通过实施改进措施,可提高现有资源利用率和工作效率,缩短预约时间和术前平均住院日,进而提高全院运行效率。

Objective To increase workload,reduce unit cost,and improve hotpital economic effieiency.Methods From the perspective of cost behavior analysis,the digestive endoscopy was taken as an example for analysis by distinguishing between fixed and variable costs and classifying the cost of site,equipment and manpower etc.Based on sufficient analysis of the use of costs,a program for resource allocation,performance management and information construction were formulated and implemented.Results The workload sharply increased by 31.56%in the first half of 2017;the appointment time of general gastroscopy and anaesthetized gastroscopy respectively reduced to 1-2 days and 2-5 days respectively.Furthermore,the preoperative average length of stay declined from 5.64 days to 3.9 days.Conclusion The cost behavior analysis has good applicability in medical examination efficiency management.By implementing improvement measures,it can improve the utilization rate of existing resources and work efficiency,shorten the appointment time and average hospital stay before surgery,and thus improve the operation efficiency of the whole hospital,thus improve the efficiency of whole hospital.
