子宫腺肌病相关脑梗死的临床特点分析 被引量:5

摘要 目的:初步探讨子宫腺肌病相关脑梗死患者的临床特点及可能发病机制。方法:筛查2003年1月至2018年8月在广西医科大学第一附属医院治疗的子宫腺肌病患者5570例,其中子宫腺肌病合并脑梗死患者11例(0.20%),平均年龄(43.8±5.6)岁。回顾分析子宫腺肌病合并脑梗死患者的临床表现、实验室及影像学检查等临床资料。结果:11例子宫腺肌病合并脑梗死患者均为急性起病,有不同程度言语不清、肢体瘫痪、麻木等局灶性神经功能缺失的症状和体征。其中7例(63.6%)缺少传统脑梗死危险因素。脑梗死发病急性期内,患者颅脑MRI弥散加权成像(DWI)显示,脑内出现单一高信号病灶5例(45.5%),出现2个及以上不同动脉供血区多发高信号病灶6例(54.5%)。血液生化检查显示,外周血D-二聚体水平明显升高8例(72.7%),血浆癌抗原CA125水平升高10例(90.9%),患者均出现不同程度贫血。结论:子宫腺肌病相关脑梗死以患者缺少脑梗死危险因素、脑内出现多发性梗死灶为特点,其发生机制可能与患者外周血D-二聚体、CA125水平升高导致血液凝固性升高有关。
