膝痛方联合中药熏蒸对慢性膝关节痛的治疗效果及对疼痛因子的影响研究

Effects on chronic knee joint pain and pain factors the treated with the xitong decoction and Chinese herbal fumigation

目的探讨膝痛方联合中药熏蒸对慢性膝关节痛的治疗效果分析及疼痛因子的影响。方法选择2014年6月至2017年1月收治的99例慢性膝关节痛患者,按照随机数字表法分组,对照组49例予以膝痛方治疗,治疗组50例予以膝痛方+中药熏蒸治疗。观察比较两组临床疗效、不良反应及疼痛因子[肿瘤坏死因子-α(TNF-α)、环氧化酶2(COX-2)、胰岛素样生长因子-1(IGF-1)]水平情况,对疼痛情况[视觉模拟评分法(VAS评分)]、膝关节功能[膝关节功能评分(HSS评分)]及生活质量[生活质量量表评分(QO评分)]进行统计。结果治疗组治疗总有效率为92. 00%,高于对照组69. 39%,差异有统计学意义(P <0. 05)。治疗组患者治疗1个月后TNF-α、COX-2水平低于对照组,IGF-1水平高于对照组,差异有统计学意义(P <0. 05)。治疗组治疗1个月后VAS评分低于对照组,HSS、QOL评分高于对照组,差异有统计学意义(P <0. 05)。两组不良反应发生率比较,差异无统计学意义(P> 0. 05)。结论膝痛方联合中药熏蒸治疗慢性膝关节痛,效果显著,安全性高,可缓解患者关节疼痛度,改善其关节功能及生活质量。

Objective To explore the effects on chronic knee joint pain and pain factors the treated with the xitong decoction and Chinese herbal fumigation.Methods From June 2014 to January 2017,99 cases of chronic knee joint pain were collected and divided into two groups according to the random number table.In the control group,49 cases were treated with xitong formula.In the treatment group,50 cases were treated with the combined therapy of xitong formula and Chinese herbal fumigation.The clinical efficacy,adverse reaction and pain factors(TNF-α,COX-2 and IGF-1)were observed and compared between the two groups.The pain situation(VAS score),knee joint function(HSS score)and living quality(QOL score)were analyzed statistically.Results The total effective rate was 92.00% in the treatment group,higher than 69.39% in the control group,indicating the significant difference(P<0.05).After 1-month treatment,the levels of TNF-α and COX-2 in the treatment group were lower than the control group and IGF-1 was higher than the control group,indicating the significant differences(P<0.05).After 1-month treatment,VAS score in the treatment group was lower than the control group,HSS and QOL scores were higher than the control group,indicating the significant differences(P<0.05).The difference was not significant in the incidence of the adverse reactions between the two groups(P >0.05).Conclusionp The combined treatment of xitong formula and Chinese herbal fumigation achieves the significant effects on chronic knee joint pain with high safety.This therapy relieves joint pain and improves the joint function and quality of life in the patients.
