MRI动态增强扫描联合DTI对前列腺中央区良恶性结节的诊断价值

Value of DCE-MRI combined with DTI in the diagnosis of benign and malignant nodules at the central zone of prostate

目的 :探讨MRI动态增强扫描(DCE-MRI)、DTI联合检查对前列腺中央区良恶性结节的诊断价值。方法 :选取行前列腺MRI扫描的50例泌尿外科患者,共发现59个结节,其中前列腺癌(PCa)结节20个(PCa组),良性前列腺增生(BPH)结节39个(BPH组),均行T2WI、DCE及DTI扫描,观察检查结果。结果:PCa组的ADC值显著低于BPH组,各向异性分数值均显著高于BPH组(均P<0.05),2组峰值时间值及曲线斜率值比较差异均有统计学意义(均P<0.05),而强化率比较差异无统计学意义(P> 0.05);2组TIC比较差异有统计学意义(P<0.05);DTI、DCE及两者联合诊断良恶性结节的特异度均> 80%,DCE扫描的敏感度较低,3种诊断技术的ROC曲线下面积比较,联合诊断最大。结论:DTI与DCE联合检查对前列腺中央区良恶性结节的诊断具有更高的临床价值,值得推广应用。

Objective:To investigate the clinical significance of dynamic contrast enhance(DCE)MRI combined with DTI in the diagnosis of benign and malignant nodules in the central zone of prostate.Methods:50 patients from urinary surgery were selected.There were 59 nodules,of which 20 malignant nodules were set as prostate cancer(PCa)group and 39 benign nodules were set in benign prostatic hyperplasis(BPH)group.All of them were scanned by T2WI,DCE and DTI,and the results were observed.Results:Compared with the BPH group,the ADC value of the PCa group was significantly lower and the FA value was significantly higher(all P<0.05).There were significant differences in the TTP and R values between the two groups,but there was no significant difference in the SImax.There was a significant difference in the TIC curve between the two groups(P<0.05).The specificity of DTI,DCE and two combined diagnosis of benign and malignant nodules was >80%,and the sensitivity of DCE scan was low.The AUC of three diagnostic techniques was compared,DTI and DCE were relatively small,and the joint diagnosis of AUC was the largest.Conclusion:DTI combined with DCE in diagnosis of benign and malignant nodules in the central zone of prostate has higher clinical value,and has the value of popularization.
