溃结康丸联合美沙拉嗪缓释片治疗脾虚湿蕴型慢性非特异性溃疡性结肠炎的临床疗效及对患者血清白细胞介素8、肿瘤坏死因子α的影响

目的观察溃结康丸联合美沙拉嗪缓释片治疗脾虚湿蕴型慢性非特异性溃疡性结肠炎(CUC)的临床疗效及对患者血清白细胞介素8(IL-8)、肿瘤坏死因子α(TNF-α)的影响。方法将64例脾虚湿蕴型CUC患者按照随机数字表法分为2组。对照组32例予常规西药美沙拉嗪缓释片口服治疗;观察组32例在对照组治疗基础上加用溃结康丸治疗。2组均治疗1个月后统计疗效,并观察治疗前后中医证候评分、生活质量评分及血清中IL-8、TNF-α水平变化情况。结果观察组临床疗效、胃镜疗效优于对照组(P <0. 05)。2组治疗后舌红边有齿痕、食少纳差、肢体困倦、黏液脓血便、脘腹胀满、腹泻便溏评分均降低(P<0. 05),且观察组各项评分降低优于对照组(P <0. 05)。2组治疗后生活质量评分均提高(P <0. 05),且观察组升高明显优于对照组(P <0. 05)。2组治疗后血清中IL-8、TNF-α水平均明显下降(P <0. 05),且观察组下降优于对照组(P <0. 05)。结论溃结康丸联合美沙拉嗪缓释片治疗脾虚湿蕴型CUC疗效确切,并可改善生活质量,促进中医症状改善。

Objective To observe the clinical effects of Kuijiekang pill combined with mesalazine sustained-release tablets in the treatment of chronic nonspecific ulcerative colitis(CUC)with spleen deficiency and dampness accumulation type and its effect on serum interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α).Methods 64 patients with spleen deficiency and dampness accumulation type of CUC were divided into two groups according to random number table.32 cases in control group were treated by oral administration of conventional western medicine mesalazine sustained-release tablets.32 cases in observation group were treated by Kuijiekang pills on the basis of the control group.The curative effect of two groups was evaluated after one month of treatment,and the changes of TCM symptom score,quality of life score and serum IL-8 and TNF-αlevels were observed before and after treatment.Results The clinical efficacy of the observation group was better than that of the control group(P<0.05).After treatment,the scores of dental marks on the red edge of tongue,poor appetite,limb sleepiness,mucous pus and blood stool,abdominal distension and diarrhea and feces in the two groups were all decreased(P<0.05),and the scores in the observation group were better than those in the control group(P<0.05).After treatment,the scores of quality of life in both groups were improved(P<0.05),and the increase in the observation group was significantly better than that in the control group(P<0.05).After treatment,the serum levels of IL-8 and TNF-αin both groups decreased significantly(P<0.05),and the decrease in the observation group was better than that in the control group(P<0.05).Conclusion Kuijiekang pill combined with mesalazine sustained-release tablets is effective in the treatment of CUC with spleen deficiency and dampness accumulation type,and can improve the quality of life and promote the improvement of TCM symptoms.
