大剂量美托洛尔对慢性充血性心力衰竭患者心脏功能的影响

Effects of High-dose Metoprolol on Cardiac Function in Patients with Chronic Congestive Heart Failure

目的:探讨大剂量美托洛尔治疗慢性充血性心力衰竭的临床效果及对患者心脏功能的影响。方法:选择2017年1月—2018年1月我院收治的50例慢性充血性心力衰竭患者为研究对象,其中22例给予小剂量美托洛尔治疗(小剂量组),28例给予大剂量美托洛尔治疗(大剂量组)。对两组患者的临床治疗效果以及心脏功能指标[心脏指数(CI)、心输出量(CO)和左心做功(LCW)]进行评价,同时比较两组不良反应发生率。结果:大剂量组治疗总有效率为89. 29%(25/28),显著高于小剂量组的77. 27%(17/22),差异有统计学意义(P <0. 05);治疗前两组患者心脏功能指标CI、CO、LCW无显著差异(P> 0. 05),治疗后均改善且大剂量组患者CI、CO、LCW显著优于小剂量组,差异有统计学意义(P <0. 05);两组患者不良反应发生率比较差异无统计学意义(P> 0. 05)。结论:适当增大美托洛尔剂量有助于提高慢性充血性心力衰竭治疗效果,改善患者心脏功能,且不明显增加不良反应。

Objective:To investigate the clinical effect of high-dose Metoprolol on chronic congestive heart failure and its influence on cardiac function.Methods:50 patients with chronic congestive heart failure admitted to our hospital from January 2017 to January 2018 were selected as study subjects.22 patients were given low-dose Metoprolol(low-dose group)and 28 patients were given high-dose metoprolol(high-dose group).The clinical efficacy and cardiac function indexes(CI,CO and LCW)of the two groups were evaluated,and the incidence of adverse reactions was compared.Results:The effective rate of high-dose group was 89.29%(25/28),significantly higher than that of low-dose group 77.27%(17/22),and the dif6ference was statistically significant(P<0.05).There was no significant difference in CI,CO and LCW between the two groups before treatment(P >0.05).After treatment,the indexes of CI,CO and LCW in the high-dose group were significantly better than those in the low-dose group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P >0.05).Conclusion:Appropriate increase of metoprolol dosage is helpful to improve the therapeutic effect of chronic congestive heart failure,as well as improve cardiac function,and does not increase adverse reactions.
