固定-活动义齿联合修复牙列缺损的临床效果

Clinical effect of fixed-removable combination denture in repairing dentition defect

目的探讨固定-活动义齿联合修复牙列缺损的临床效果。方法选取100例牙列缺损患者,根据修复方案不同分为研究组和对照组,各50例。对照组给予活动义齿修复治疗,研究组给予固定-活动义齿联合修复治疗。修复前及修复1年后采用口腔健康影响程度(OHIP-14)量表评价口腔健康相关生活质量(包括口腔疼痛、身体机能、心理状况和独立能力4个方面);于修复前、修复2年后评价两组患者的牙龈出血指数评分;修复2年后评估两组患者的修复效果;修复1年后评估两组患者满意度。结果修复1年后,两组患者的口腔疼痛评分、身体机能评分、心理状况评分、独立能力评分和OHIP-14总分均较修复前下降(均P <0. 05),研究组患者的口腔疼痛评分、身体机能评分、独立能力评分及OHIP-14总分均低于对照组(均P <0. 05),但两组的心理状况评分比较,差异无统计学意义(P> 0. 05)。修复后2年,两组患者的牙龈出血指数评分均较修复前降低,且研究组低于对照组(均P <0. 05);两组患者牙列缺损修复效果及修复总有效率比较,差异无统计学意义(均P> 0. 05)。研究组患者对牙列缺损修复的满意度优于对照组(P <0. 05),对修复的总满意度高于对照组(P <0. 05)。结论固定-活动义齿联合修复牙列缺损疗效显著,可改善患者的口腔健康状况及生活质量,并有助于提高患者的满意度。

Objective To explore the clinical effect of fixed-removable combination denture in repairing dentition defect.Methods A total of 100 patients with dentition defect were selected and divided into study group or control group according to different restoration schemes,with 50 cases in each group.The control group was given active denture repair treatment,and the study group was given fixed-active combination denture repair treatment.Before and 1 year after repair,the Oral Health Impact Pro le-14(OHIP-14)score was used to assess oral health-related quality of life(including oral pain,bodily functions,psychological status,and independence);before and 2 years after restoration,gingival hemorrhage index scores of two groups were evaluated;2 years after repair,repair effect of two groups was evaluated,and 1 year after repair,satisfaction rate of two groups was evaluated.Results One year after repair,oral pain score,physical function score,psychological status score,independent ability score,and OHIP-14 score total score decreased in both groups(all P<0.05),with oral pain score,body function score,independent ability score,and OHIP-14 score lower in the study group than in the control group(all P<0.05),but no significant difference in psychological status score was observed between the two groups(P >0.05).Two years after repair,gingival bleeding index scores decreased in both groups,with the score lower in the study group(all P<0.05),but no significant difference existed in repairing effect or total effective rate between the two groups(P >0.05).Satisfaction rate in term of dentition defect repair was better,and total satisfaction rate was higher in the study group than in the control group(all P<0.05).Conclusion Therapeutic effect of fixed-removable combination denture in dentition defect repair is significant,for it can improve oral health status and life quality of patients,and is conducive to improving patient satisfaction.
