尿白蛋白/尿肌酐比值与2型糖尿病患者颈动脉硬化斑块形成的相关性

Correlation between urine albumin to creatinine ratio and carotid atherosclerotic plaque formation in patients with type 2 diabetes mellitus

目的探讨尿白蛋白/尿肌酐比值(UACR)与2型糖尿病(T2DM)患者颈动脉硬化(CAS)斑块形成的相关性。方法根据UACR水平将329例T2DM患者分为正常组(UACR <30 mg/g) 226例、白蛋白尿组(UACR≥30 mg/g) 103例。收集患者的一般资料,检测相关生化指标,应用彩色多普勒超声检查所有患者的颈动脉,并比较两组CAS斑块形成的发生率。采用Logistic回归模型分析T2DM患者CAS斑块形成的影响因素。结果白蛋白尿组CAS斑块的发生率为90. 29%,高于正常组的53. 98%(P <0. 05)。UACR、年龄、血清尿酸均是T2DM患者CAS斑块形成的独立危险因素(均P <0. 05)。结论 UACR水平升高的T2DM患者CAS斑块的发生风险升高。当T2DM患者UACR≥30 mg/g时,应注重CAS斑块的筛查及诊治。

Objective To investigate the correlation between urinary albumin to creatinine ratio(UACR)and carotid atherosclerosis(CAS)plaque formation in patients with type 2 diabetes mellitus(T2DM).Methods According to the level of UACR,329 patients with T2DM were divided into normal group(UACR<30 mg/g,n=226)or albuminuria group(UACR≥30 mg/g,n=103).Clinical data of all patients were collected,related biochemical indices were detected,carotid arteries were examined by color Doppler ultrasound,and incidence rate of CAS plaque formation was compared between the two groups.Logistic regression model was used to analyze the influencing factors of CAS plaque formation in T2DM patients.Results The incidence rate of CAS plaque was higher in the albuminuria group than in the normal group(90.29%vs.53.98%,P<0.05).UACR,age,and serum uric acid were the independent risk factors for the CAS plaque formation in T2DM patients(all P<0.05).Conclusion The risk of CAS plaque formation increases in T2DM patients with UACR levels increasing.When UACR is≥30 mg/g,attention should be paid to the screening,diagnosis,and treatment for CAS plaque in T2DM patients.
