CT、MRI诊断膝关节隐性骨折的效果比较

Effect comparison of CT and MRI in the diagnosis of occult fracture of the knee

目的比较CT和MRI诊断膝关节隐性骨折的效果。方法选取2013年3月～2017年11月因疼痛就诊于惠州市中大惠亚医院放射科或中山大学附属第一医院放射科的68例临床疑似膝关节隐性骨折患者作为研究对象。所有患者在1周内接受CT和MRI检查,以手术诊断和临床随访结果作为诊断标准,比较CT和MRI对膝关节隐性骨折诊断的敏感度、特异度和准确度。结果有61例患者为膝关节隐性骨折,7例为其他原因导致临床疼痛。CT、MRI诊断的敏感度比较,差异无统计学意义(P>0.05),MRI诊断的特异度高于CT诊断,差异有统计学意义(P>0.05),MRI诊断的准确度高于CT诊断,差异有统计学意义(P>0.05)。结论 MRI对膝关节隐性骨折患者的诊断效果优于CT诊断。

Objective To compare the effect of computed tomography(CT)and magnetic resonance imaging(MRI)in the diagnosis of occult fracture of the knee.Methods From March 2013 to November 2017,68 patients with clinically suspected occult fracture of the knees were enrolled in the Department of Radiology of Huizhou Zhongda Huiya Hospital or the First Affiliated Hospital of Sun Yat-sen University.All patients underwent CT and MRI within 1 week.The results based on surgical diagnosis and clinical follow-up were used as the diagnostic criteria to compare the sensitivity,specificity and accuracy of CT and MRI in the diagnosis of occult fractures of the knee.Results There were 61 subjects in total definitely diagnosed as occult fracture of the knee,and the rest 7 ones suffered from clinical pain due to other reasons.There was no statistical difference in the sensitivity of CT and MRI diagnosis(P >0.05).The specificity of MRI diagnosis was higher than that of CT diagnosis with a significant difference(P<0.05).The accuracy of MRI diagnosis was also higher compared with that of CT with statistical significance(P<0.05).Conclusion MRI is superior to CT in the diagnosis of patients with the occult fracture of the knee.
