动态心电图与常规心电图对冠心病心律失常的检出率比较

Comparison of the detection rate of dynamic electrocardiogram and routine electrocardiogram on arrhythmia in coronary heart disease

目的比较动态心电图与常规心电图对冠心病心律失常的检出率。方法将118例冠心病患者作为研究对象,均进行动态心电图与常规心电图检查。以冠状动脉造影结果为金标准。比较两种心电图对冠心病心律失常的诊断效果。结果常规心电图检出冠心病78例,误诊14例,漏诊4例,灵敏度为95.1%,特异度为61.1%,准确率为84.7%;动态心电图检出冠心病81例,误诊4例,漏诊1例,灵敏度为98.8%,特异度为88.9%,准确率为95.8%。动态心电图对心律失常的阳性检出率高于常规心电图(P<0.05)。动态心电图对室性早搏成对、室性早搏二三联律、房性早搏成对、房性早搏二三联律及短阵室上速的检出例数多于常规心电图(P<0.05)。结论动态心电图对冠心病心律失常的检出率较常规心电图更高,检验结果更准确,对冠心病心律失常诊断有积极意义。

Objective To compare detection rate of dynamic electrocardiogram and routine electrocardiogram on the arrhythmia in coronary heart disease.Methods A total of 118 patients with coronary heart disease were selected as study objects.All patients were examined by dynamic electrocardiogram and routine electrocardiogram.The results of coronary angiography were taken as gold standard.The diagnostic effects of two electrocardiograms on arrhythmia of coronary heart disease were compared.Results Routine electrocardiogram detection showed that there were 78 cases of coronary heart disease,14 cases of misdiagnosis,4 cases of missed diagnosis,with sensitivity of 95.1%,specificity of 61.1%,accuracy of 84.7%.Dynamic electrocardiogram detection showed that there were 81 cases of coronary heart disease,4 cases of misdiagnosis,1 case of missed diagnosis,with sensitivity of 98.8%,specificity of 88.9%,accuracy of 95.8%.The positive rate of the dynamic electrocardiogram in detecting arrhythmia was higher than that of the routine electrocardiogram(P<0.05).The numbers detected by the dynamic electrocardiogram for paired premature ventricular beats,premature ventricular beats,premature atrial beats,premature atrial beats and supraventricular tachycardia were more than those by the routine electrocardiogram(P<0.05).Conclusion The detection rate of arrhythmia of coronary heart disease by dynamic electrocardiogram is higher than that of conventional electrocardiogram,and the test results are more accurate,and it has positive significance in diagnosis of arrhythmia of coronary heart disease.
