胫骨高位截骨治疗轻中度内侧间室病变膝关节骨性关节炎的临床效果

Clinical effect of high tibial osteotomy in the treatment of mild to moderate medial compartment lesion knee osteoarthritis

目的探析在轻中度内侧间室病变膝关节骨性关节炎的临床治疗中应用胫骨高位截骨的临床效果。方法选取2017年1月至2018年1月在我院接受诊治的68例轻中度内侧间室病变膝关节骨性关节炎患者为研究对象,按照随机数字表法将其分为参照组和研究组,每组34例。参照组患者实施腓骨近端截骨术进行治疗,研究组患者实施胫骨高位截骨进行治疗。比较两组患者的临床治疗效果。结果研究组治疗总有效率为94.12%,明显高于参照组的76.47%,差异具有统计学意义(P<0.05)。治疗后,两组患者的HSS评分均显著升高,且研究组明显高于参照组,差异具有统计学意义(P<0.05)。治疗后,两组患者的VAS评分均显著降低,且研究组明显低于参照组,差异具有统计学意义(P<0.05)。研究组患者术后并发症总发生率为2.94%,明显低于参照组的20.59%,差异具有统计学意义(P<0.05)。结论胫骨高位截骨治疗轻中度内侧间室病变膝关节骨性关节炎的效果显著,其可显著改善患者膝关节功能,缓解其疼痛程度,减少术后并发症的发生,值得在临床上推广应用。

Objective To explore the clinical effect of high tibial osteotomy in the treatment of mild to moderate medial compartment lesion knee osteoarthritis.Methods A total of 68 patients with mild to moderate medial compartment lesion knee osteoarthritis treated in our hospital from January 2017 to January 2018 were selected as the research objects.They were divided into reference group and study group according to the random number table method,with 34 cases in each group.The reference group was treated with proximal fibular osteotomy,while the study group was treated with high tibial osteotomy.The clinical effects of the two groups were compared.Results The total effective rate of treatment in the study group was 94.12%,which was significantly higher than 76.47%in the reference group,and the difference was statistically significant(P<0.05).After treatment,the HSS scores of the two groups significantly increased,and that in the study group was significantly higher than the reference group,the difference was statistically significant(P<0.05).After treatment,the VAS scores of the two groups significantly decreased,and that in the study group was significantly lower than the reference group,the difference was statistically significant(P<0.05).The total incidence of postoperative complications in the study group was 2.94%,which was significantly lower than 20.59%in the reference group,and the difference was statistically significant(P<0.05).Conclusion The high tibial osteotomy in the treatment of mild to moderate medial compartment lesion knee osteoarthritis has remarkable effect,which can significantly improve knee function,relieve pain of patients,and decrease the incidence of complications.It is worth popularizing and applying in clinic.
