基于中足三柱理论应用伊里扎洛夫技术治疗开放Lisfranc损伤的临床研究

Clinical study of Irizalov technique in the treatment of open Lisfranc injury based on the three-column theory of middle-foot

目的探讨基于中足三柱理论应用伊里扎洛夫技术治疗开放Lisfranc损伤的临床效果。方法选择2012年至2017年收治的22例开放性Lisfranc损伤患者为研究对象,患者均实施基于中足三柱理论、伊里扎洛夫技术的微型锁定钢板外置固定手术治疗。术后对患者进行随访,并分析其伤口愈合情况及手术疗效。结果 22例患者均获得随访,随访时间为12～48个月,平均(19.3±1.7)个月;患者骨性愈合时间为9～14周,平均(11.7±2.1)周。所有患者均未发生严重并发症。末次随访时,患者的治疗优良率为86.4%。结论基于中足三柱理论、伊里扎洛夫技术的微型锁定钢板外置固定治疗开放性Lisfranc损伤的临床效果显著,有利于患者早期进行功能锻炼,且其低切迹、小轮廓外形、固定装置取出简单等优势为患者的术后康复带来了极大的方便,值得临床推广应用。

Objective To investigate the clinical effect of Irizalov technique on open Lisfranc injury based on the threecolumn theory of middle-foot.Methods Twenty-two patients with open Lisfranc injury admitted from 2012 to 2017 were selected as the study objects.All patients underwent external fixation with mini-locking plate surgical treatment based on the three-column theory of middle-foot and Irizalov technique.The patients were followed up after operation,and the wound healing and surgical effect were analyzed.Results The 22 patients were followed up for 12-48 months,with average of(19.3±1.7)months.The bone healing time of the patients was 9-14 weeks,with average of(11.7±2.1)weeks.No serious complications occurred in all patients.At the last follow-up,the excellent and good rate of treatment was 86.4%.Conclusion External fixation with mini-locking plate based on the three-column theory of middle-foot and Irizalov technique has a significant clinical effect on open Lisfranc injury,which is conducive to early functional exercise,and its advantage of low notch,small contour shape and simple removal of fixator have brought great convenience to patients'postoperative rehabilitation,which is worthy of clinical promotion and application.
