血清CK-MB、SMb联合检测在中老年急性心肌梗塞早期诊断中的应用效果

Application of combined detection of serum CK-MB and SMb in early diagnosis of acute myocardial infarction in middle-aged and elderly patients

目的探讨血清心肌酸激酶同工酶(CK-MB)和血清肌红蛋白(SMb)联合检测在中老年急性心肌梗塞(AMI)早期诊断中的应用价值。方法选取2015年5月至2017年9月我院收治的急性胸痛疑似AMI患者120例,按诊断结果将其分为AMI组(65例,AMI患者)和非AMI组(55例,非AMI但有胸痛症状患者)。比较两组血清CK-MB及SMb的水平,评估其诊断价值。结果 AMI组CK-MB水平在0～2 h达到阳性标准,并在6～8 h内维持较高水平;SMb水平在0～2 h达到阳性标准,并在3～5 h内维持较高水平。AMI组患者CK-MB、SMb水平均高于非AMI组(P<0.05)。联合诊断的灵敏度和特异度均高于CK-MB、SMb单独检测(P<0.05)。结论血清CK-MB和SMb联合检测对AMI早期临床诊断效果较好,值得临床推广。

Objective To explore the application value of combined detection of serum myocardial acid kinase isoenzyme(CK-MB)and serum myoglobin(SMb)in early diagnosis of acute myocardial infarction(AMI)in the middleaged and elderly patients.Methods A total of 120 patients with acute chest pain suspected AMI admitted to our hospital from May 2015 to September 2017 were enrolled and divided into AMI group(65 patients with AMI)and non-AMI group(55 patients with non-AMI but with chest pain symptoms)according to the diagnostic results.The levels of serum CK-MB and SMb were compared between the two groups,and their diagnostic value was evaluated.Results In the AMI group,the levels of CK-MB reached the positive standard at 0-2 h,and maintained a high level within 6-8 h;the levels of SMb reached the positive standard at 0-2 h,and maintained a high level within 3-5 h.The levels of CK-MB and SMb in the AMI group were higher than those in the non-AMI group(P<0.05).The sensitivity and specificity of combined detection were higher than those of single detection of CK-MB or SMb(P<0.05).Conclusion Combined detection of serum CK-MB and SMb has a good clinical diagnosis in the clinical early diagnosis of AMI.
