Reverse Sural Nerve Vascular Pedicle Flap for Repairing Deep Ankle Joint Wounds

目的:总结应用逆行腓肠神经血管轴形皮瓣修复踝部深度创面的相关技术问题,提高手术效果,避免术后并发症。方法:回顾性分析2010年3月-2016年10月笔者科室收治的8例应用逆行腓肠神经血管轴形皮瓣修复踝部深度创面患者,归纳总结术中相关技术问题,对手术设计、注意事项及并发症发生情况予以分析。结果:本组共8例,术后7例切口一期愈合,1例出现皮瓣下渗液,冲洗引流后皮瓣愈合,创面封闭;1例并发供瓣区植皮部分皮肤坏死。8例足外侧区感觉麻木(3个月后均自行恢复),术后无感染等并发症发生。结论:应用逆行腓肠神经血管轴形皮瓣修复踝部深度创面,安全可靠,疗效显著。

Objective To summarize the relative technical problems of using reverse gastrocnemius axial skin flap to repair deep ankle joint wounds in order to improve the performance of the operation and avoid postoperative complications. Methods Retrospective analysis of 8 patients with deep ankle joint wounds repaired with reverse sural nerve vascular pedicle flap admitted to our department from March 2010 to October 2016. Summarized the related technical problems during the operation. Surgical design, precautions and complications were analyzed. Results There were 8 cases in this group. 7 cases were healed after operation. One case showed skin flap infiltration, flushing and drainage, skin flap healing and wound closure. Partial skin necrosis of donor site in one case. 8 cases felt numbness in the lateral region of the foot (all recovered spontaneously after 3 month). No infection and other complications after the operation. Conclusion It is safe and reliable to use reverse gastrocnemius axial skin flap to repair the deep wound of ankle.
