ω-3鱼油脂肪乳对SIRS患者的治疗效果及作用机制

Therapeutic effects and action mechanism of ω-3 fish oil fat emulsion on SIRS

目的探讨ω-3鱼油脂肪乳治疗全身炎性反应综合征(SIRS)患者的临床效果及其作用机制。方法选取2014年1月至2016年1月在重症监护室治疗的51例SIRS患者,根据病历号单双号分为研究组26例和对照组25例。2组患者均给予全胃肠外营养,研究组同时给予ω-3鱼油脂肪乳,治疗5 d后对比2组的临床效果及相应指标。结果治疗前,2组患者的IgA、IgG、IgM、CRP、TNF-α、IL-1、IL-6水平、APACHEⅡ评分比较差异无统计学意义(P> 0. 05);治疗后,2组患者的血清IgA、IgG、IgM水平较治疗前显著的提高(P <0. 05),CRP、TNF-α、IL-1、IL-6水平、APACHEⅡ评分较治疗前显著的降低(P <0. 05);治疗后,研究组的血清IgA、IgG、IgM水平显著的高于对照组(P <0. 05),CRP、TNF-α、IL-1、IL-6水平、APACHEⅡ评分显著的低于对照组(P <0. 05)。2组MODS发生率、28 d病死率比较差异无统计学意义(P> 0. 05)。结论ω-3鱼油脂肪乳治疗SIRS患者对免疫学水平的改善具有一定的作用,对患者的治疗具有一定的临床意义。

Objective To investigater the therapeutic effects and action mechanism ofω-3 fish oil fat emulsion on systemic inflammatory response syndrome(SIRS).Methods A total of 51 patients with SIRS who were treated in ICU of our hospital from January 2014 to January 2016 were enrolled in the study.According to the medical record number(odd or even numbers).these patients were divided into observation group(n=26)and control group(n=26).The patients in both groups were treated by total parenteral nutrition,moreover,the patients in observation group were givenω-3 fish oil fat emulsion.After 5-day treatment the clinical curative effects and corresponding indexes were observed and compared between the two groups.Results Before treatment,there were no significant differences in the serum levels of IgA,IgG,IgM,CRP,TNF-α,IL-1,IL-6,and APACHEⅡscores between two groups(P >0.05).After treatment,the serum levels of IgA,IgG,IgM were significantly increased in both groups(P<0.05),however,the levels of CRP,TNF-α,IL-1,IL-6 and APACHEⅡscores were significantly decreased,as compared with those before treatment(P<0.05),moreover,the levels of IgA,IgG,IgM in observation group were significantly higher than those in control group(P<0.05),however the levels of CRP,TNF-α,IL-1,IL-6,and APACHEⅡscores were significantly lower than those in control group(P<0.05).There were no significant differences in incidence rate of MODS and 28-day fatality rate between two groups(P >0.05).Conclusion Theω-3 fish oil fat emulsion in treatment of SIRS can improve immunological levels at some extent,which has a certain clinical significance for the treatment of patients.
