生长抑素联合大黄对急性胰腺炎患者血清淀粉酶、TNF-α及TREM-1水平的影响

Effects of somatostatin combined with rhubarb on the levels of serum amylase,TNF-α and TREM-1 in patients with acute pancreatitis

目的探讨生长抑素联合大黄对急性胰腺炎患者血清淀粉酶、肿瘤坏死因子-α(TNF-α)及髓样细胞触发受体-1(TREM-1)水平的影响。方法选取2016年3月至2017年3月收治的132例急性胰腺炎患者,根据随机数字法分为对照组和观察组,每组66例。对照组患者采用生长抑素治疗,观察组患者采用生长抑素联合大黄治疗,分析2组患者治疗效果。结果观察组患者腹痛缓解时间、体温、血淀粉酶、排便恢复时间和禁食时间均短于对照组(P <0. 05)。治疗前,2组患者血清淀粉酶、TBil、血钙、TNF-α、TREM-1水平和APACHE-Ⅱ评分比较差异无统计学意义(P> 0. 05)。治疗后3 d、7 d时,观察组患者血清淀粉酶、TBil、TNF-α、TREM-1水平和APACHE-Ⅱ评分均低于对照组,血钙水平均高于对照组(P <0. 05)。结论生长抑素联合大黄治疗急性胰腺炎可加快患者临床症状的消失,抑制机体炎性因子的表达,调节患者血钙、TBil水平。

Objective To investigate the effects of somatostatin combined with rhubarb on the levels of serum amylase,tumor necrosis factor alpha (TNF-α) and myeloid cell triggering receptor-1 (TREM-1) in patients with acute pancreatitis. MethodsA total of 132 patients with acute pancreatitis who were admitted and treated in our hospital from March 2016 to March 2017 were enrolled in the study. According to random digital table, these patients were divided into control group ( n =66) and observation group ( n =66).The patients in control group were treated by somatostatin,however,the patients in observation group were treated by somatostatin combined with rhubarb.After treatment the therapeutic effects were observed and compared between the two groups. ResultsThe abdominal pain relief time,the recovery time of body temperature,serum amylase and defaecation as well as fasting time in observation group were significantly shorter than those in control group ( P <0.05). Before treatment there were no significant differences in the levels of serum amylase,serum calcium,TBil,TNF-α, TREM-1 and APACHE-Ⅱscores between two groups ( P >0.05).At 3d,7d after treatment the levels of serum amylase,TBil,TNF-α,TREM-1 level and APACHE-Ⅱscore in observation group were significantly lower than those in control group,however,the serum calcium levels in observation group were significantly higher than those in control group ( P < 0.05). ConclusionSomatostatin combined with rhubarb in treatment of acute pancreatitis can speed up the disappearance of clinical symptoms,inhibit the expression of inflammatory factors,regulate the serum levels of calcium and TBil of patients.
