孕前检查联合孕期保健对高龄产妇妊娠结局的影响

Effect of Pre-pregnancy Examination and Prenatal Health Care on Pregnancy Outcome of Advanced Maternal Age

目的:探究孕前检查联合孕期保健对高龄产妇妊娠结局的影响。方法:选取2016年1-12月在本院接受孕期检查的产妇300例。依据是否接受孕前检查将其分为对照组(n=150)和研究组(n=150)。对照组接受孕期保健,研究组在对照组基础上接受孕前检查。比较两组产妇不同时间节点的心理状态(SDS、SAS评分),比较两组妊娠结局、妊娠并发症及新生儿健康状况。结果:研究组妊娠3周、6个月及分娩后6 h的SDS、SAS评分均明显低于对照组(P<0.01);研究组早产、宫内窘迫、难产、早夭或胎死腹中率均低于对照组(P<0.05),阴道顺产率高于对照组(P<0.05),两组剖宫产率比较差异无统计学意义(P>0.05);研究组未出现妊娠期糖尿病、胎膜早破、贫血等情况,且胎膜早破、胎位异常、产后出血、妊娠期高血压的发生率均低于对照组(P<0.05);研究组新生儿体重及Apgar评分均高于对照组(P<0.05)。结论:孕前检查联合孕期保健可调节高龄产妇心理舒适度,减少妊娠期并发症发生风险,改善妊娠结局,值得临床大力推广。

Objective:To explore the effect of pre-pregnancy examination and prenatal health care on pregnancy outcome of advanced maternal age.Method:A total of 300 pregnant women who underwent pre-pregnancy examination in our hospital from January to December 2016 were selected.According to whether they received pre-pregnancy examination or not,they were divided into control group(n=150)and research group(n=150).The control group received prenatal health care,while research group received pre-pregnancy examination on the basis of control group.The mental status(SDS and SAS)of two groups at different time points were compared,the pregnancy outcome,complications and health status of newborn between two groups were compared.Result:The SDS and SAS scores of study group at 3 weeks,6 months of pregnancy and 6 hours after delivery were significantly lower than those of control group(P<0.01).The median rates of premature delivery,intrauterine distress,dystocia,early death or fetal death in study group were lower than those of control group(P<0.05),and the vaginal spontaneous rate was higher than that of control group(P<0.05).The cesarean section rate in two groups was compared,the difference was not statistically significant(P >0.05).There were no gestational diabetes mellitus,premature rupture of membranes and anemia in study group,and the incidence of premature rupture of membranes,abnormal fetal position,postpartum hemorrhage and pregnancy-induced hypertension in study group were lower than those of control group(P<0.05).The neonatal weight and Apgar score in study group were higher than those of control group(P<0.05).Conclusion:Pre-pregnancy examination combined with pregnant health care can adjust the psychological comfort of advanced maternal age,reduce the risk of complications during pregnancy and improve pregnancy outcomes,which is worthy of clinical vigorous promotion.
