摘要
目的探讨孕妇血清缺氧诱导因子1a(HIF-1a)和血红素加氧酶1(HO-1)水平与子痫前期以及妊娠结局的关系。方法选取128例子痫前期孕妇为研究对象,其中轻度子痫前期60例,重度子痫前期68例;早发型子痫前期(发病<32孕周)48例,晚发型子痫前期(发病>32周)80例。以同期正常妊娠孕妇100例为对照组。采用ELISA法检测孕妇血清HIF-1a、HO-1水平,同时分析HIF-1a、HO-1水平与子痫分型以及产后预后的关系。结果对照组血清HIF-1a和HO-1分别为56.35±8.36 ng/L,466.78±89.75 ng/L;子痫前期组HIF-1a和HO-1分别为72.64±10.64ng/L,348.46±136.69ng/L,差异有统计学意义(P <0. 01);轻度子痫前期组与重度子痫前期组相比差异也具有统计学意义(P <0. 05)。比较子痫前期的分类与妊娠结局,两者在分娩孕周,新生儿体重以及胎盘重量均存在显著差异。结论孕妇血清HIF-1a和HO-1变化与子痫前期严重程度与预后有关,尤其早发型预后较差。晚发型子痫前期孕妇及时检测血清HIF-1a和HO-1水平,可望为临床子痫前期的早发现,早干预和早治疗提供依据。
Objective To investigate the relationship between levels of serum HIF-la and HO-1 and preeclampsia and pregnancy outcomes of pregnant women. Methods 128 pregnant women with preeclampsia were selected as the research subjects,including 60 cases of mild preeclampsia,68 cases of severe preeclampsia,48 cases of early onset preeclampsia(<32 weeks of gestation),and 80 cases of late onset( > 32 weeks of gestation).100 cases of normal pregnant women in the same period were selected as the control group.ELISA method was used to detect the levels of HIF-la and HO-1 in the serum of all pregnant women.The relationship between the level of these serum markers and the clinical characteristics of preeclampsia was then analyzed. Results Levels of serum HIF-la and HO-1 in the control group were 56.35±8.36ng/L, 466.78± 89.75 ng/L,respectively.The HIF-la and HO-1 in pre-eclampsia group were 72.64±10.64ng/L,348.46±136.69ng/L,respectively,and there was significant difference between the two groups( P < 0.01 ).The statistical difference also exists between mild pre-eclampsia group and severe pre-eclampsia group( P <0.05).When we analyzed the classification of preeclampsia and pregnancy outcome ,there were significant differences in the two groups( including delivery pregnancy week,birth weight and placental weight. Conclusion The changes of serum hypoxia-induced factor la and heme oxygenase 1 in pregnant women are related to the severity and prognosis of pre-eclampsia,especially the early onset prognosis is poorer.Early detection of serum HIF-la and HO-1 in the late pregnancy will provide the information for early detection,early intervention and early treatment.
