盆底治疗用于妇科开腹手术患者的价值探讨

Discussion on the value of pelvic floor therapy for open surgery of gynecology

目的:比较妇科开腹手术后采用盆底治疗与传统的围手术期处理方法的安全性、有效性、患者的满意度。方法:2017年1月-2017年12月在广东省江门市人民医院进行开腹手术的患者共82例,其中48例术后24小时即开始盆底治疗,34例术后24小时开始采用传统的围手术期处理方法。两组进行对照,比较两组患者的术后住院时间,恢复胃肠排气时间,停止静脉营养能量输液时间,术前后基础代谢中白蛋白下降的情况以及两组术后并发症的发生情况。结果:盆底治疗与传统的围手术期组相比,前组术后首次肠道排气的时间提前,停止静脉营养能量输液时间提前,患者恢复自主活动的时间提前,术后复查基础代谢中的白蛋白下降情况减轻,术后住院时间缩短。两组术后的并发症发生率差异无明显意义。结论:妇科开腹手术的患者应在术后24小时进行盆底治疗,该方法安全、有效、利于患者术后胃肠功能恢复的同时,减少了患者的住院时间及治疗费用,大大提高了患者的满意度。

Objective:To compare the safety,effectiveness and patient satisfaction of pelvic floor treatment after open surgery with traditional perioperative treatment.Methods:From January 2017 to December 2017,a total of 82 patients underwent open surgery in people’s hospital of Jiangmen city,Guangdong province,of which 48 patients began pelvic floor treatment 24 hours after surgery and 34 patients began to use traditional perioperative treatment 24 hours after surgery.The two groups were compared for postoperative hospital stay,recovery of gastrointestinal exhaust time,cessation of intravenous nutrient energy infusion,reduction of albumin in basic metabolism before and after surgery,and occurrence of postoperative complications in the two groups.Results:Compared with the traditional perioperative group,the first postoperative intestinal exhaust time in the former group was advanced,the time to stop intravenous nutrient energy infusion was advanced,the time for the patient to resume independent activity was advanced,the albumin decrease in basic metabolism was reduced after postoperative review,and the postoperative hospitalization time was shortened.There was no significant difference in postoperative complication rate between the two groups.Conclusion:Patients with gynecological laparotomy should receive pelvic floor treatment 24 hours after surgery.This method was safe,effective and beneficial to the recovery of gastrointestinal function after surgery,while reducing the hospitalization time and treatment costs of patients,greatly improving the satisfaction of patients.
