医院人员支出与药品耗材占比倒挂机制若干问题研究

Study on Several Problems of the Inversion Mechanism of Hospital Personnel Expenditure and Drug Consumable Proportion

目的明确倒挂机制建立必要的量关系,并就机制建立提出建议。方法普查收集资料,用SPSS建立数据库并进行统计分析,采用散点图描述相关指标间的变化趋势。结果同类医院药品耗材占比目标值,如省三级综合医院药品、耗材为41.28%、14.65%;不同类型医院药品、耗材占比调节系数范围存在差异,业务量每年平均间距变化对应耗材占比增减幅度也有差异;人员支出占比与药品耗材占比近似呈反方向变动趋势;分析出人员支出占比与药品耗材占比倒挂机制的要素关系。结论倒挂机制建立诸多问题需关注;倒挂机制建立可一院一策,保障人员支出相应增加的资金来源和稳定,建立防范高价药、高值耗材使用的反向激励机制,各项制度与措施协同开展。

Objective To identify the necessary quantity relations of the establishment of inversion mechanism,and put forward relevant suggestions on the establishment of the inversion mechanism.Methods Collecting data by census,establishing a database with SPSS and performed statistical analysis,and using scatter plots to describe the trends among relevant indicators.Results The target value of drug and consumable proportion of the provincial tertiary general hospitals were 41.28%and 14.65%.The adjustment coefficient range of drug and consumable proportion existed difference between different types of hospitals.There was difference in the change of the amount of per average spacing traffic corresponded to mutative extent of the consumable proportion.The proportion of personnel expenditure and drug and consumable proportion showed inverse change tendency approximately.Analysis showed relations among elements of the inversion mechanism between the proportion of personnel expenditure and drug and consumable proportion.Conclusion Many problems of the establishment of the inversion mechanism should be paid attention to,a one-in-one policy to the establishment of the inversion mechanism should be established,the corresponding increase in the source of funds and stability of personnel expenditure be guaranteed,and a reverse incentive mechanism to prevent the use of high-priced drugs and high-value consumables be established and developed in coordination with the various systems and measures.
