摘要
住院医师培训体系是目前我国医疗系统采用并推广的青年医师培养体系,至今已取得较大的成效,但目前对住院医师的培养方法尚存在改进的空间。为更好地培养合格的住院医师,中南大学湘雅二医院急诊医学科自2017年起开始实施住院医师培养的"EXCELLENT——卓越计划"。"EXCELLENT"是病历汇报、每日十问、临床思维、特殊情况、倾听与告知、自主学习、辅助检查、夜班注意事项和团队合"英文首字母的缩写,涉及住院医师管理的多个层面,旨在通过这一计划培养出具有卓越临床技能的住院医师。"EXCELLENT——卓越计划"实施1年多来,取得了如下的初步成果:患者满意度度提高,急诊病房患者平均住院日缩短,科室对住院医师考核评分上升,住院医师对科室的评价提高。
The residency training program is aimed to train medicine graduates to be qualified doctors,and it is the currently adopted system in China.The program has been successfully implemented for years,but there are some defects.In order to train residents better,the Department of Emergency of The Second Xiangya Hospital launched the EXCELLENT program.EXCELLENT is the acronym for expected case presentation of seniors,X(ten)questions,clinical thinking,exceptional and emergent scenarios,listening and informing,learning by yourself,examination planning,night shift notices and teamwork.Since the implementation of EXCELLENT program,some satisfying achievements have been acquired,such as improving patients’satisfaction,shortening average length of stay(ALOS),and increasing evaluation of the residents.
