类风湿关节炎患者血清IL-27水平及外周血PLR检测与疾病活动度的相关性研究

Correlation Analysis between Serum IL-27 Levels and Peripheral BloodPLR Detection and Disease Activity in Rheumatoid Arthritis Patients

目的分析活动期类风湿关节炎(RA)患者血清白介素-27(IL-27)水平及外周血血小板/淋巴细胞比率(PLR)与疾病活动度的相关性,探讨IL-27与PLR在RA病情活动度评估中的价值。方法收集2017年10月～2018年10月风湿免疫科确诊的96例活动期RA患者,依据DAS28评分分为高度活动组32例、中度活动组34例、轻度活动组30例,同期选择在该院体检的健康者30例作为对照组,两组都采用酶联免疫吸附实验(ELISA)检测血清IL-27水平,免疫比浊法检测RA患者类风湿因子(RF)和超敏C反应蛋白(hs-CRP),化学发光法检测RA患者抗环瓜氨酸肽(CCP)抗体,魏氏法检测RA患者红细胞沉降率(ESR),分析RA患者血细胞检测并计算PLR。结果 RA患者血清IL-27水平,PLR值(17.74±8.58pg/ml,165.50±39.26)显著高于对照组(5.70±1.43pg/ml,101.49±18.80),差异有统计学意义(t=9.802～10.665,均P<0.01),且活动度越高,IL-27,PLR水平越高(P<0.05)。活动期RA患者血清IL-27水平、PLR值均与ESR,hs-CRP呈显著正相关(r^2=0.220 0～0.522 5,均P<0.01)。RA患者血清IL-27水平与PLR呈显著正相关(r^2=0.208 8,P=0.0001)。结论 RA患者血清IL-27水平与PLR联合检测对RA病情活动度评估具有潜在价值。

Objective To explore the value of IL-27 and platelet/lymphocyte ratio(PLR)in the evaluation of RA activity by analyzing the correlation between serum interleukin-27(IL-27)levels and peripheral blood platelet/lymphocyte ratio(PLR)and disease activity in rheumatoid arthritis(RA)patients at active stages.Methods Collected 96 patients with active RA diagnosed by rheumatic immunology department from October 2017 to October 2018.According to DAS28 score,they were divided into 32 highly active group,34 moderate active group and 30 mildly active group.In the same period,30 healthy patients who underwent physical examination in the hospital were selected as the control group.Serum IL-27 levels in both groups were detected by enzyme-linked immuno sorbent assay(ELISA).Rheumatoid factor(RF)and hypersensitive C-reactive protein(hs-CRP)were detected by immunoturbidimetric assay.Anti-cyclic citrulline peptide(CCP)antibodies were detected by chemiluminescent immunoassay.Erythrocyte sedimentation rate(ESR)was detected by Westergren method.Results Serum IL-27 levels and PLR value(17.74±8.58 pg/ml,165.50±39.26)in RA patients were significantly higher than healthy control group(5.70±1.43 pg/ml,101.49±18.80),the difference was statistically significant(t=9.802~10.665,P<0.01),and the higher the activity level was,the higher the IL-27 levels and PLR value were(P<0.05).Serum IL-27 levels and PLR were significantly positively correlated with ESR and hs-CRP(r^2=0.220 0~0.522 5,P<0.01).Serum IL-27 levels was significantly positively correlated with PLR in RA(r^2=0.208 8,P=0.000 1).Conclusion The combination of serum IL-27 levels and PLR in RA patients has potential value in the evaluation of RA disease activity.
