健康教育处方联合患者记录册在治疗放射性口腔黏膜炎中的应用

Application of Health Education Prescription Combined with Patient Record Book in The Treatment of Radioactive Oral Mucositis

目的探讨健康教育处方联合患者记录册干预在鼻咽癌放疗患者中的应用效果。方法将71例鼻咽癌放疗患者按随机数表法分为干预组35例和对照组36例,对照组给予放射治疗与常规护理及健康教育,干预组在此基础上使用健康教育处方联合患者记录册干预;比较两组患者的漱口正确执行率和两组患者在放疗至30 Gy和放疗结束时发生放射性口腔黏膜炎的例数。结果干预组和对照组的漱口正确执行率分别为91.4%和55.6%;干预组和对照组在放疗30 Gy时Ⅰ级口腔炎发生率分别为2.8%和28%,Ⅱ级口腔炎发生率分别为0%和2.7%。干预组和对照组在放疗结束时Ⅰ级口腔炎发生率分别为23%和47%,Ⅱ级口腔炎发生率分别为11%和28%,Ⅲ级口腔炎发生率分别为0%和5.5%;干预组患者在放疗至30 Gy(X^2=9.882,P=0.004)和放疗结束时(X^2=15.99,P=0.001)放射性口腔黏膜炎发生率显著低于对照组。结论应用健康教育处方联合患者记录册干预能提高患者的依从性及医嘱执行力,从而减轻鼻咽癌患者放射性口腔黏膜炎程度,降低放射性口腔黏膜炎的发生率。

Objective To explore the effect of health education prescription combined with patient record intervention in patients with nasopharyngeal carcinoma during radiotherapy.Methods 71 patients with nasopharyngeal carcinoma were randomly divided into intervention group(n=35)and control group(n=36).The control group was given radiotherapy,routine nursing and health education.On this basis,the intervention group used health education prescription combined with patient record book intervention.The correct execution rate of gargle and the cases of radiation oral mucositis after 30 Gy radiotherapy and at the end of radiotherapy were compared between the two groups.Results The correct execution rate of gargle in the intervention group and the control group was 91.4% and 55.6%,respectively.In the intervention group and control group,the incidence of grade Ⅰstomatitis was 2.8% and 28%,respectively,and the incidence of grade Ⅱ stomatitis was 0% and 2.7% after 30 Gy radiotherapy.At the end of radiotherapy,the incidence of grade Ⅰ stomatitis in the intervention group and control group was 23% and 47%,the incidence of grade Ⅱ stomatitis was 11% and 28%,the incidence of grade Ⅲ stomatitis was 0% and 5.5%,respectively.The incidence of radiation oral mucositis in the intervention group was significantly lower than that in the control group(P<0.05).Conclusion The application of health education prescription combined with the intervention of patient record book can improve the compliance of patients and the ability of execution of doctors' orders,thus reducing the degree of radiation oral mucositis and reducing the incidence of radioactive oral mucositis in patients with nasopharyngeal carcinoma.
