Web of Science数据库小儿方面的右美托咪定文献研究分析

A bibliometric analysis on research of dexmedetomidine in pediatrics based on Web of Science Core Collection

目的分析国际上右美托咪定(DEX)在小儿中的主要研究成果和发展趋势。方法使用Note Express软件在Web of Science核心数据集中进行文献检索,拟定检索策略[主题=("dexmedetomidine") AND主题=("pediatric*"or"child*"or"infant*"or"toddler*"or"adolescent*")],时间跨度从建库到2017年7月3日,对文献性质无特殊要求,建立Excel数据库,并对文献发表年度、刊载来源、国家/地区、作者所在机构、学科研究方向、以及研究主题、基金资助机构分布进行文献计量学分析。结果共检索到719篇文献,文献数量逐年上涨;文献刊载的来源虽然较广,但主要集中在麻醉学和危重医学;美国发文量最高(342篇,占47. 566%),总被引频次为4 829,中国位列第二,总被引频次居第三位;研究机构也绝大多数是美国的高校及附属医院或儿童医学中心,其中哈佛大学居榜首; 39个研究方向中涉及到麻醉学的占46. 732%(336篇);排名前15位的关键词大致可分为以下几类:药物(丙泊酚、氯胺酮、七氟醚、咪唑安定、可乐定); DEX作用(镇静、镇痛、麻醉、减轻躁动及撤药综合征);围手术期的应用(包括手术前,心脏手术,重症监护等);手术室外的应用(如核磁共振检查)。结论 DEX在小儿中的研究发展迅速,关注度逐年增高,基础和临床研究并重,研究热点包括与一些常规麻醉药物比较其药效学以及重症监护和手术室内、外的应用。

Objective To analyze the current research status and development trend of dexmedetomidine in pediatrics from the literature point of view.Methods Literature has been collected through Web of Science Core Collection by NoteExpress and establish database by Excel.And the search strategy was[theme=("dexmedetomidine")AND theme=("pediatric*"or"child*"or"infant*"or"toddler*"or"adolescent*")]from the establishment of database to July 3,2017.There is no special requirement for the nature of literature.The annual trend,journals distribution,country/region distribution,institution distribution,subject category distribution,key words and grants support distribution of dexmedetomidine in pediatrics were analyzed by applying literature metrological methods.Results 719 papers were indexed,which mainly were about anesthesia and intensive care medicine and the numbers grew rapidly in recent years.The United States was the most productive region(342,47.566%)and its total citations were in the forefront(4 829).China ranked the second according to quantity of paper published and the third to total citations.The top ten research institutions according to the number of published papers were almost American universities,affiliated hospitals and children health care centers and the first one was Harvard University.Nearly half of the research directions were referred to anesthesiology(336,46.732%).The related research focused on such key words:drugs(propofol,ketamine,sevoflurane,midazolam and clonidine),effects(sedation,analgesia,anesthesia,agitation and withdrawal),perioperative uses(premedication,intensive care and cardiac surgery)and outdoor applications(magnetic resonance imaging).Conclusion Dexmedetomidine in pediatrics research develops rapidly and medical doctors pay more attention on it through years.Basic and clinical researches are both developing,search focuses are on comparison of dexmedetomidine with some anesthetics in pharmacodynamics and applications inside or outside Intensive Care Units and operating room.
