新生儿血液白细胞参考区间的建立与验证

Establishment and verification of reference intervals for neonatal blood leukocytes

目的:建立和验证正常新生儿的血液白细胞参考值范围。方法:动员新生儿监护人同意参与调查活动,做必要的告知,采集血样并检测血液白细胞数据,用统计分析方法对收集到的数据进行统计分析,建立新生儿白细胞的参考区间并进行诊断效率验证。结果:新生儿白细胞数据分布不符合正态分布,需用百分位数法评估其参考区间。0～28 d新生儿组白细胞数据需进一步分组为(0,3)d、(3,21)d和(21,28)d组,且(3,21)d新生儿血液白细胞存在性别差异,其差异具有统计学意义。结论:(0,3)d新生儿白细胞参考区间(6.67～19.0)×10~9/L;(3,21)d女性新生儿白细胞参考区间(6.47～16.7)×10~9/L,(3,21)d男性新生儿白细胞参考区间(5.59～12.4)×10~9/L;(0,28)d新生儿白细胞参考区间(6.28～17.4)×10~9/L。各分组参考区间的临床诊断效率均>85%。

Objective:To establish and validate a range of blood leukocyte reference values for normal newborns.Methods:Neonatal guardians were mobilized to participate in the investigation,to give necessary information,to collect blood samples and detect the data of leukocytes.The data collected were analyzed by statistical analysis,and the reference interval of leukocytes was established and the diagnostic efficiency was verified.Results:The distribution of WBC data in newborns did not conform to normal distribution,and the reference interval should be evaluated by percentile method.The white blood cell data of 0~28 d neonates should be further grouped into(0,3)d,(3,21)d and(21,28)d groups,respectively,and(3,21)d neonatal white blood cells in the presence of gender differences were statistically significant.Conclusion:The reference interval of neonatal leukocytes in(0,3)days was(6.67~19.0)×109/L.(3,21)d female newborn white blood cell reference area was(6.47~16.7)×109/L,(3,21)d male newborn white blood cell reference interval was(5.59~12.4)×109/L.(0,28)d neonatal leukocyte reference interval was(6.28~17.4)×109/L.The clinical diagnostic efficiency of each group reference interval >85%.
