腹腔镜与开腹卵巢囊肿剥除术对机体免疫功能和术后卵巢功能的影响

Effects of laparoscopic and open ovarian cystectomy on immune function and postoperative ovarian function

目的探讨腹腔镜与开腹卵巢囊肿剥除术对机体免疫功能和术后卵巢功能的影响。方法选取2012年12月～2017年12月我院收治的204例卵巢囊肿患者,随机分为对照组和实验组各102例,对照组患者给予开腹卵巢囊肿剥除术,实验组患者应用腹腔镜卵巢囊肿剥除术,比较两组患者免疫功能、卵泡储备功能、卵巢功能的差异。结果术后实验组CD3+(54.72±6.38)%、CD4+(39.96±4.95)%、CD8+(27.75±3.11)%、CD4+/CD8+(1.52±0.21)%较对照组均明显提高(P<0.05);实验组患侧术后3个月窦状卵泡数量(7.26±1.56),患侧术后6个月(8.08±1.76)显著高于对照组(P<0.05);术后3个月实验组FSH(7.65±0.71)nIU/mL、LH(6.87±0.95)nIU/mL、E2(122.43±16.35)pg/mL同对照组比较均明显改善(P<0.05)。结论针对卵巢囊肿患者应用腹腔镜卵巢囊剥除术,可提高患者免疫功能,提高患者卵巢功能,促进患者恢复,值得推广应用。

Objective To investigate the effects of laparoscopic and open ovarian cystectomy on immune function and postoperative ovarian function.Methods 204 patients with ovarian cysts admitted in our hospital from December 2012 to December 2017 were randomLy divided into control group and experimental group,with 102 cases in each group.The control group was treated with open ovarian cystectomy.The experimental group was treated with laparoscopic ovarian cystectomy.The differences in immune function,follicular reserve function,and ovarian function between the two groups were compared.Results The postoperative CD3+(54.72±6.38)%,CD4+(39.96±4.95)%,CD8+(27.75±3.11)%,CD4+/CD8+(1.52±0.21)%in the experimental group were significantly higher than those of the control group(P<0.05).The number of sinusoid follicles of the affected side in the experimental group was(7.26±1.56)at 3 months after operation,and the number of sinusoid follicles(8.08±1.76)in the affected side was significantly higher than that in the control group(P<0.05).FSH(7.65±0.71)nIU/mL,LH(6.87±0.95)nIU/mL,and E2(122.43±16.35)pg/mL in the experimental group were significantly improved compared with those of the control group at 3 months after surgery(P<0.05).Conclusion Laparoscopic ovarian cystectomy for patients with ovarian cysts can improve the immune function of patients,improve the ovarian function of patients and promote the recovery of patients.It is worthy of popularization and application.
